纳入标准：

Inclusion criteria

排除标准：

1.体检数据缺失率＞20%；
2.患有恶性肿瘤、终末期心衰、终末期肾病等严重共病；
3.基线时已处于CKM3-4期；
4.随访期间失访或死亡（无结局记录）；

Exclusion criteria：

1.Missing rate of physical examination data >20%;
2. Having severe comorbidities such as malignant tumors, end-stage heart failure, end-stage kidney disease, etc.
3.Already in CKM stage 3-4 at baseline;
4. Lost to follow-up or death during follow-up (no outcome records).

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

在对社区病历和体检数据库的回顾提取数据时，获得了新都区卫健局授权，将数据进行拷贝，拷贝过程中将数据进行了脱敏处理，去除个人身份识别信息，确保数据无法关联到具体个人；数据处理过程全程在加密环境下进行，配备专业数据安全管理团队。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

During the retrospective extraction of data from the community medical records and physical examination database, authorization was obtained from the Xindu District Health and Wellness Bureau to copy the data. During the copying process, the data was de-identified, removing personally identifiable information to ensure that the data could not be linked to specific individuals; the data processing was conducted entirely in an encrypted environment, with a professional data security management team in place.
