研究目的：

比较胃癌患者行常规D2根治术第12组淋巴结清扫和经右侧肋缘下入路行肝门系膜清扫的安全性、并发症及预后差异。  

Objectives of Study：

To compare the safety, complications and prognosis of conventional D2 radical resection of lymph node dissection and right subcostal approach for hilar mesangial dissection in patients with gastric cancer.

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

1. 术前存在肝功能异常者，包括转氨酶及胆红素异常； 2. 存在腹腔镜手术禁忌者，如严重心肺功能不全者； 3. 既往多次行腹盆腔手术或腹腔广泛粘连者； 4. 合并幽门梗阻、胃穿孔、胃出血等需急诊手术的患者； 5. 术前影像学提示有腹膜癌结节种植及转移者； 6. ASA 分级 >= IV 级和/或 ECOG 体力状态评分 >= 2 分； 7. 具有严重的心肺功能、凝血功能障碍或合并严重的基础疾病不能耐受手术者； 8. 具有严重的精神疾病病史； 9. 怀孕或哺乳期妇女； 10. 术前有感染未控制者； 11. 有研究者认为的其他临床、实验室检查检验异常不宜参加该项试验； 12. 患者及家属拒绝签署知情同意书。

Exclusion criteria：

1. Patients with abnormal liver function before operation, including abnormal transaminase and bilirubin; 2. Patients with contraindications to laparoscopic surgery, such as severe cardiopulmonary dysfunction; 3. Patients who have undergone abdominopelvic surgery or extensive abdominal adhesions several times in the past; 4. Patients with pyloric obstruction, gastric perforation, gastric bleeding and other emergency surgery; 5. Preoperative imaging suggests peritoneal cancer nodular implantation and metastasis; 6. ASA grade >= IV and/or ECOG performance status score >= 2; 7. Patients with severe cardiopulmonary function, coagulation dysfunction or combined with severe underlying diseases cannot tolerate surgery; 8. Having a history of severe mental illness; 9. Pregnant or lactating women; 10. Patients with uncontrolled infection before surgery; 11. Other clinical and laboratory abnormalities considered by the investigator should not participate in the trial; 12. Patients and their families refused to sign informed consent.
