研究目的：

（1）描述小儿先天性肾积水及泌尿系结石主要照顾者电子健康素养、疾病进展恐惧与随访依从性的得分现状。 （2）分析主要照顾者电子健康素养对随访依从性的直接影响。 （3）构建中介效应模型，检验疾病进展恐惧在电子健康素养与随访依从性之间的中介路径是否成立。  

Objectives of Study：

(1) Describe the current score status of electronic health literacy, disease progression fear, and follow-up compliance among primary caregivers of children with congenital hydronephrosis and urinary tract stones. (2) Analyze the direct impact of primary caregivers' electronic health literacy on follow-up compliance; (3) Construct a mediation effect model, to test whether the intermediary path of truedisease progression fear between electronic health literacy and follow-up compliance holds is valid

纳入标准：

Inclusion criteria

排除标准：

1. 患儿处于危重症抢救期、生命体征极不稳定； 2.患儿合并有严重的先天性认知障碍、智力发育迟滞或神经系统疾病； 3. 照顾者既往有明确的精神病史，或目前正处于严重的心理应激状态； 4. 照顾者患有严重的视力、听力障碍或认知功能障碍，无法配合数据采集； 5. 受雇佣的专业护工、短期育儿嫂或临时替代的远亲； 6. 问卷填写缺失值超过20%或存在明显逻辑前后矛盾者。

Exclusion criteria：

1. The child is in a critical rescue period with extremely unstable vital signs; 2. The child has severe congenital cognitive impairment, intellectual developmental delay, or neurological disorders; 3. The caregiver has a clear history of mental illness or is currently in a state of severe psychological stress; 4. The caregiver suffers from severe visual, hearing impairment, or cognitive dysfunction and cannot cooperate with data collection; 5. Employed professional caregivers, short-term nannies, or temporarily substituted distant relatives; 6. Questionnaires with missing values exceeding 20% or showing obvious logical inconsistencies.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究采用横断面调查研究设计，拟选取三级医院专科门诊及随访的先天性肾积水或泌尿系结石患儿主要照顾者。通过单中心大样本便利抽样收集数据,使用EpiData 3.1软件双人录入数据，并进行一致性校验和逻辑核查。所有数据仅用于本研究分析，不用于其他用途。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

This study was adoptsed a cross-sectional survey design， the primary caregivers for children with congenital hydronephrosis or urinary stones who were selected in specialized outpatient clinics and followed up in tertiary hospitals. double person entry data of convenient sampling with a single center large sample with EpiData 3.1 software, and conduct consistency and logical checks. All data will be used for this study.
