主要研究目的： 手术与颌垫治疗对中老年女性ADD髁突骨改建的影响。 次要研究目的： 手术与颌垫对下颌运动功能、生活质量和疼痛症状的影响。  

Objectives of Study：

Primary objective: To evaluate the effects of surgery versus splint therapy on condylar bone remodeling in middle-aged and elderly female patients with ADD. Secondary objectives: To evaluate the effects of surgery and splint therapy on mandibular mobility, quality of life, and pain symptoms.

纳入标准：

Inclusion criteria

排除标准：

1. 合并其他严重系统性疾病者，如类风湿性关节炎、糖尿病等； 2. 既往有颞下颌关节手术史者、创伤史； 3. 恶性肿瘤和终末期疾病患者； 4. 精神疾病患者。

Exclusion criteria：

1. Patients with other severe systemic diseases, such as rheumatoid arthritis, diabetes, etc.; 2. Patients with a history of previous temporomandibular joint surgery or trauma; 3. Patients with malignant tumors or terminal illnesses; 4. Patients with mental/psychiatric disorders.

关节盘复位缝合术（DR）。性质：外科手术治疗。通过耳前切口或经关节镜进入颞下颌关节上腔，松解关节盘前附着，将前移位的关节盘向后复位至正常解剖位置，并使用不可吸收缝线将其缝合固定于关节囊后壁。旨在恢复正常的盘-髁关系。

Intervention：

Disc Repositioning (DR). Intervention Type: Surgical Treatment. Procedure Details: The upper compartment of the temporomandibular joint (TMJ) is accessed via a preauricular incision or an arthroscopic approach. Following the release of the anterior attachment, the anteriorly displaced articular disc is retropositioned to its normal anatomical location. It is then firmly secured to the posterior joint capsule utilizing non-absorbable sutures. This procedure aims to restore the normal physiological disc-condyle relationship.
