研究目的：

本研究旨在针对妊娠期高血糖防控难题，建立一个包含夫妻双方、贯穿“备孕-怀孕-分娩-新生儿-儿童”全孕育时间轴、涵盖多民族人群具有重庆区域特色的妊娠期高血糖专病队列；从暴露组整体视角系统评估孕前环境暴露与孕前糖尿病、糖尿病前期和妊娠期糖尿病等妊娠期高血糖疾病的发生风险，系统刻画重庆地区育龄女性的孕前暴露组特征谱；全面同等识别影响夫妻双方的环境-社会-心理-行为等因素，聚焦环境内分泌物干扰物、心理、营养与节律等关键暴露，解析不同妊娠期高血糖的遗传易感特征及主要环境危险因素，深入阐明其通过代谢重编程与免疫稳态失衡驱动该病形成的机制，为妊娠期高血糖孕期血糖监测与管理新技术的临床实践提供新的循证医学依据。

Objectives of Study：

This study aims to address the challenges of preventing and controlling gestational hyperglycemia by establishing a specialized cohort for gestational hyperglycemia that includes both partners, spans the entire reproductive timeline from “preconception to pregnancy, delivery, the neonatal period, and childhood,” covers a multi-ethnic population, and reflects the regional characteristics of Chongqing; From a population-level perspective, we will systematically evaluate the association between preconception environmental exposures and the risk of developing gestational hyperglycemia-related conditions—including pre-gestational diabetes, prediabetes, and gestational diabetes—and systematically map the preconception exposure profiles of childbearing-age women in Chongqing; comprehensively and equitably identify environmental, social, psychological, and behavioral factors affecting both partners, with a focus on key exposure factors such as environmental endocrine disruptors, psychological, nutritional, and circadian factors; to analyze the genetic susceptibility and primary environmental risk factors associated with different types of gestational hyperglycemia; to elucidate the mechanisms by which metabolic reprogramming and immune homeostasis imbalance drive disease development; and to provide new evidence-based medical foundations for the clinical application of novel technologies in the monitoring and management of gestational hyperglycemia.

排除标准：

1.有明确的泌尿生殖系统疾病史，如不孕不育、睾丸/卵巢发育不良综合征、生殖系统外伤、列于孕优项目调查表内的泌尿生殖系统炎症或性传播疾病；
2.青春期后腮腺炎，或医学诊断的遗传病史；
3.参加项目之前三个月内男性有高烧病史。

Exclusion criteria：

1.A definite history of urogenital system diseases, such as infertility, testicular/ovarian dysplasia syndrome, reproductive system trauma, urogenital system inflammation or sexually transmitted diseases listed in the survey form of the pregnancy-preferential project;
2.Mumps after puberty, or a medically diagnosed genetic disorder;
3.History of high fever in the male within three months prior to participation in the project.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：	数据采集采用电子化病例报告表和电子数据采集系统。孕期和围产期资料通过医院电子病历系统导出，3–6岁随访问卷、量表评分、体格测量和实验室结果由经过培训的调查员现场录入平板端，系统实时逻辑校验并提示缺失值，减少人工录入误差。
Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：	Data collection was performed using electronic case report forms and an electronic data capture system. Pregnancy and perinatal data were exported from the hospital's electronic medical record system. For the 3–6-year follow-up, questionnaires, scale scores, physical measurements, and laboratory results were entered on-site into tablets by trained investigators, with real-time logical validation and missing value prompts in the system to reduce manual entry errors.
