研究目的:

利用超高场7T MRI的高信噪比,高空间分辨率,探索颈椎脊髓成像技术方案;并开展DCM高分辨率颈脊髓成像,通过发展定位、定性、定量描述脊髓微观结构病变方法,发现脊髓毫米/亚毫米级病灶,探索其与患者症状、预后的关系,为理解DCM脊髓微结构病变机制、促进DCM精确诊治提供有力工具。  

Objectives of Study:

Taking advantage of the high signal-to-noise ratio (SNR) and high spatial resolution of ultra-high field 7T magnetic resonance imaging (MRI), this study aims to explore the technical scheme for cervical spinal cord imaging. Meanwhile, high-resolution cervical spinal cord imaging of degenerative cervical myelopathy (DCM) will be carried out. By developing methods for localizing, qualifying, and quantifying microstructural lesions of the spinal cord, millimetric/submillimetric lesions in the spinal cord will be identified. Furthermore, the relationships between these lesions and patients' symptoms as well as prognosis will be explored, so as to provide a powerful tool for understanding the mechanism of spinal cord microstructural lesions in DCM and promoting the accurate diagnosis and treatment of DCM.

纳入标准:

Inclusion criteria

排除标准:

1.健康对照组: ⑴存在脊髓功能障碍的症状或体征; ⑵有脊髓损伤或手术史; ⑶颈椎MRI扫描存在脊髓受压变形; 2.DCM术后患者组: ⑴外伤后脊髓功能障碍急性加重; ⑵合并脑、胸腰椎或外周神经病变; ⑶怀疑与手术相关的神经损伤或C5神经根麻痹; ⑷因金属内植物、幽闭恐惧等原因无法进行7T MRI扫描。 3.DCM术前患者组: (1)颈椎外伤性骨折脱位或外伤后脊髓功能障碍加重的患者; (2)颈椎感染性病变; (3)颈椎肿瘤; (4)全身存在不可控制的活动性感染; (5)身体有其他疾病不允许进行手术治疗; (6)不具备自主行为能力; (7)神经根型或混合型患者,合并严重胸、腰椎神经压迫或合并其它脑、脊髓、外周神经病变,可能干扰临床评估的患者; (8)既往颈椎手术史,干扰临床评估的患者;

Exclusion criteria:

1.Healthy control group: (1) Symptoms or signs of spinal cord dysfunction; (2) History of spinal cord injury or surgery; (3) Cervical MRI scan shows compression and deformation of the spinal cord; 2.DCM Postoperative patient group: (1) Acute exacerbation of spinal cord dysfunction after trauma; (2) Combined brain, thoracolumbar, or peripheral neuropathy; (3) Suspected nerve damage or C5 nerve root paralysis related to surgery; (4) Due to reasons such as metal plants and claustrophobia, 7T MRI scans cannot be performed. 3.DCM Preoperative patient group: (1) Patients with cervical traumatic fractures and dislocations or aggravated spinal cord dysfunction after trauma; (2) Cervical infectious lesions; (3) Cervical spine tumors; (4) Uncontrollable active infections exist throughout the body; (5) Surgical treatment is not allowed for other illnesses in the body; (6) Lack of autonomous behavioral ability; (7) Patients with nerve root type or mixed type, combined with severe thoracic and lumbar nerve compression or other brain, spinal cord, peripheral nerve lesions that may interfere with clinical evaluation; (8) Patients with a history of cervical spine surgery that interferes with clinical evaluation;
