研究目的:

本研究旨在通过一项为期4周、多中心、随机对照的临床试验,评估与验证基于“中枢-外周-中枢”闭环的脑机接口踝关节康复训练,相较于常规康复训练,对改善脑卒中患者踝关节主动背屈活动度的临床疗效与优效性。  

Objectives of Study:

This study aims to evaluate and validate the clinical efficacy and superiority of central–peripheral–central closed-loop brain–computer interface ankle rehabilitation training compared with conventional rehabilitation training in improving active dorsiflexion range of motion of the ankle joint in stroke patients, through a 4-week, multicenter, randomized controlled clinical trial.

纳入标准:

Inclusion criteria

排除标准:

1.存在严重的心、肺、肝、肾功能不全,或未控制的高血压、糖尿病等全身性疾病,不适合进行康复训练。 2.合并其他可能影响运动功能的中枢或周围神经系统疾病(如帕金森病、重症肌无力、脊髓损伤等),或患侧下肢存在严重的关节挛缩、畸形、骨折未愈合、疼痛或皮肤破损。 3.有癫痫病史或颅内金属植入物(不含牙齿填充物);头部有严重感染或大面积创伤未愈,无法佩戴脑电采集设备。 4.存在严重的认知障碍、失语症、忽视症或精神疾病,无法理解和执行训练指令。 5. 预计无法完成既定康复疗程,或同时参与其他可能干扰本研究结果的康复或药物临床试验。 6.经研究者评估,存在任何其他可能增加风险或干扰研究评估的情况。

Exclusion criteria:

1. Presence of severe cardiac, pulmonary, hepatic, or renal insufficiency, or uncontrolled systemic diseases such as hypertension or diabetes mellitus, making the patient unsuitable for rehabilitation training. 2. Comorbidity with other central or peripheral nervous system disorders that may affect motor function (e.g., Parkinson's disease, myasthenia gravis, spinal cord injury, etc.), or presence of severe joint contracture, deformity, unhealed fracture, pain, or skin breakdown on the affected lower limb. 3. History of epilepsy or presence of intracranial metal implants (excluding dental fillings); severe infection or large unhealed wound on the scalp that prevents wearing of electroencephalography (EEG) acquisition equipment. 4. Presence of severe cognitive impairment, aphasia, neglect syndrome, or psychiatric disorders that impair the ability to understand and follow training instructions. 5. Expected inability to complete the prescribed rehabilitation course, or concurrent participation in another rehabilitation or drug clinical trial that may interfere with the results of this study. 6. Any other condition that, in the investigator's assessment, may increase risk or interfere with study evaluations.

由与本试验无直接关联的独立统计人员,使用统计软件为每个研究中心生成独立的随机分配序列。每个序列采用随机排列的固定区组(区组长度为4或6),以保证在研究过程中,任一中心内试验组与对照组的受试者数量始终保持大致平衡。

Randomization Procedure (please state who generates the random number sequence and by what method):

A statistician independent of this trial will generate separate randomization sequences for each study center using statistical software. Each sequence will use fixed blocks with random permutations (block lengths of 4 or 6) to ensure that the number of participants in the experimental group and the control group remains approximately balanced within each center throughout the study.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

1.核对病例报告表:检查其完整性及有无逻辑性错误。若有错漏,登记、复核。 2.录入:采用双人核对录入法将研究数据录入数据库。 3.数据库检查:随机抽取10%的录入数据与病例报告表原始数据进行完全核对检查,再运用描述性统计分析与探索性分析检查录入数据的完整性与逻辑性。若有错漏,登记、更正、复核。 4.保存、备份数据库,封盲

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

1. Check the case report form: check its completeness and whether there are logical errors. If there are errors and omissions, register and review. 2. Input: The research data was entered into the database using the two-person check input method. 3. Database check: 10% of the input data were randomly selected and completely checked with the original data of the case report form, and then descriptive statistical analysis and exploratory analysis were used to check the integrity and logic of the input data. If there are errors and omissions, register, correct and review.
