研究目的:

1. 母婴微生态与胎儿畸形的病例对照研究 (1)探讨胎儿畸形妊娠孕妇肠道微生态及血液、尿液代谢谱的特征。 (2)明确畸形胎儿妊娠附属物(胎盘、胎膜、脐带及羊水)的代谢与微生态特征。 (3)探究畸形胎儿或新生儿肠道微生态、血斑特征。 (4)探究不良妊娠事件如子痫前期等妊娠期疾病和自然流产、早产等不良妊娠结局的母婴微生态特征。 2. 母亲阴道菌群种植对剖宫产婴儿肠道菌群、神经发育及过敏性疾病影响的随机对照试验 (1)明确VMT对剖宫产婴儿肠道菌群发育轨迹及其代谢产物的长期修复效果。 (2)量化VMT对剖宫产婴儿神经发育迟缓风险的预防效果。 (3)量化母亲VMT对剖宫产婴儿社会-情绪发育迟缓风险的预防效果。 (4)量化VMT对剖宫产婴儿过敏性疾病(特异性皮炎、过敏性哮喘、过敏性鼻炎和食物过敏)风险的预防效果。 (5)量化VMT对剖宫产新生儿黄疸的预防效果。 (6)进一步验证VMT的安全性,全面评估该技术在临床应用中的安全效果。  

Objectives of Study:

1. Case-Control Study: (1) To characterize the gut microecology as well as the metabolic profiles of blood and urine in pregnant women with fetal malformations. (2) To define the metabolic and microecological features of gestational adnexa (placenta, fetal membranes, umbilical cord, and amniotic fluid) in pregnancies with fetal malformations. (3) To investigate the gut microecology and blood spot characteristics of fetuses or neonates with malformations. (4) To investigate maternal-infant microecological features in adverse pregnancy events, including preeclampsia, spontaneous abortion, and preterm birth. 2. Randomized Controlled Trial: (1) To determine the long-term restorative effects of vaginal microbiota transfer (VMT) on the developmental trajectory of gut microbiota and their metabolites in cesarean-born infants. (2) To quantify the preventive effect of VMT on the risk of neurodevelopmental delay in cesarean-born infants. (3) To quantify the preventive effect of maternal VMT on the risk of social-emotional developmental delay in cesarean-born infants. (4) To quantify the preventive effect of VMT on the risk of allergic diseases (atopic dermatitis, allergic asthma, allergic rhinitis, and food allergy) in cesarean-born infants. (5) To quantify the preventive effect of VMT on neonatal jaundice in cesarean-born infants. (6) To further validate the safety of VMT and comprehensively evaluate its clinical safety profile.

排除标准:

母婴微生态与胎儿畸形的病例对照研究: 1.多胎妊娠; 2.孕妇患有精神或智力障碍无法配合; 3.孕妇或直系亲属(祖父母、父母、配偶、兄弟姐妹、子女)有遗传性疾病史; 4.孕期检测出性传播疾病(艾滋病、梅毒、生殖器单纯疱疹病毒、沙眼衣原体、淋病奈瑟菌、HPV); 5.孕妇现患或既往有恶性肿瘤病史; 6.招募时孕妇存在消化系统急性病症; 7.采集标本孕妇2周内服用过微生态调节剂,6周内使用过抗生素; 8.孕妇长期服用非甾体类抗炎药,如阿司匹林等。 母亲阴道菌群种植对剖宫产婴儿肠道菌群、神经发育及过敏性疾病影响的随机对照试验: 1. 有心、肝、肾等严重内科疾病,或有家族遗传、代谢疾病史; 2. 本次妊娠患有严重产科合并症及并发症; 3. 孕期检测出性传播疾病(艾滋病、梅毒、生殖器单纯疱疹病毒、沙眼衣原体、淋病奈瑟菌、HPV); 4. GBS阳性或患滴虫性阴道炎、念珠菌性阴道炎、细菌性阴道炎等阴道炎症; 5. 入院时阴道PH > 4.5; 6. 患传染性疾病或感染性疾病; 7. 患严重精神疾病或心理障碍; 8. 孕期有服用特殊药物史、放射线接触史、吸毒或酗酒史者; 9. 见红和胎膜早破(仅限剖宫产); 10. 阴道试产失败转剖宫产; 11. 新生儿1分钟Apgar评分小于7分; 12. 新生儿出生畸形、染色体异常,出生时颅内出血或宫内感染等; 13. 新生儿出生体重 < 2500 g 或 > 4000 g。

Exclusion criteria:

Case-Control Study on Maternal-Infant Microecology and Fetal Malformations: 1.Multiple pregnancy; 2.Pregnant women with mental or intellectual disorders that preclude cooperation; 3.The pregnant woman or her immediate family members (grandparents, parents, spouse, siblings, children) have a history of genetic disorders; 4.Detection of sexually transmitted diseases during pregnancy (HIV, syphilis, genital herpes simplex virus, Chlamydia trachomatis, Neisseria gonorrhoeae, HPV); 5.The pregnant woman currently has malignant tumors or has a history of malignant tumors; 6.The pregnant woman has acute digestive system disorders at the time of enrollment; 7.The pregnant woman has taken microecological modulators within 2 weeks or antibiotics within 6 weeks prior to specimen collection; 8.The pregnant woman has long-term use of non-steroidal anti-inflammatory drugs (NSAIDs), such as aspirin. Randomized controlled trial of maternal vaginal microbiota transfer on gut microbiota, neurodevelopment, and allergic diseases in cesarean-delivered infants: 1. Presence of severe medical diseases involving the heart, liver, kidneys, etc., or a family history of genetic or metabolic disorders; 2. Presence of severe obstetric comorbidities or complications during the current pregnancy; 3. Detection of sexually transmitted diseases during pregnancy (HIV, syphilis, genital herpes simplex virus, Chlamydia trachomatis, Neisseria gonorrhoeae, HPV); 4. Positive for group B Streptococcus (GBS) or diagnosed with vaginal infections such as trichomonal vaginitis, candidal vaginitis, or bacterial vaginosis; 5. Vaginal pH > 4.5 at admission; 6. Presence of contagious or infectious diseases; 7. Presence of severe psychiatric disorders or psychological disturbances; 8. History of taking special medications, exposure to radiation, drug abuse, or alcohol abuse during pregnancy; 9. Vaginal bleeding or premature rupture of membranes (for cesarean section only); 10. Failed trial of vaginal delivery requiring conversion to cesarean section; 11. Newborn with a 1-minute Apgar score < 7; 12. Newborn with congenital malformations, chromosomal abnormalities, intracranial hemorrhage at birth, or intrauterine infection, etc.; 13. Newborn birth weight < 2500 g or > 4000 g.
