研究目的：

本研究的主要目的是在拟行下肢关节置换术的老年患者中，探讨在侧卧位下蛛网膜下腔注射0.75%布比卡因复合芬太尼后，药物血药浓度变化与麻醉平面之间的内在关联。具体来说，研究旨在明确血药浓度变化是否与麻醉平面的形成时间、最高阻滞平面、麻醉效果持续时间等临床麻醉特征相关，为老年患者下肢关节置换术的个体化麻醉方案提供依据。  

Objectives of Study：

The main purpose of this study is to investigate the intrinsic relationship between changes in drug plasma concentration and the level of anesthesia in elderly patients undergoing lower limb joint replacement surgery in the lateral position after subarachnoid injection of 0.75% bupivacaine combined with fentanyl. Specifically, the study aims to clarify whether changes in plasma drug concentration are related to clinical anesthetic characteristics such as the onset time of the anesthetic level, the highest level of blockade, and the duration of anesthetic effect, providing a basis for individualized anesthesia regimens for elderly patients undergoing lower limb joint replacement surgery.

纳入标准：

Inclusion criteria

排除标准：

1. 年龄 < 65 岁，ASA 分级 IV-V 级； 2. 合并严重心血管、呼吸系统疾病，凝血功能障碍，低蛋白血症（白蛋白 < 30 g/L）； 3. 对布比卡因、芬太尼或局部麻醉药过敏者； 4. 术前长期服用阿片类药物、肝药酶诱导 / 抑制药者； 5. BMI > 30 kg/m^2，合并精神疾病或认知障碍无法配合评估者； 6. 有腰椎手术史、椎管内肿瘤、神经系统疾病病史者。

Exclusion criteria：

1. Age < 65 years old, ASA grade IV-V; 2. Combined with severe cardiovascular and respiratory diseases, coagulation dysfunction, hypoproteinemia (albumin < 30 g/L); 3. Those who are allergic to bupivacaine, fentanyl or local anesthetics; 4. Those who have taken opioids and liver drug enzyme inducing/inhibiting drugs for a long time before surgery; 5. BMI > 30 kg/m^2, combined with mental illness or cognitive impairment who cannot cooperate with the assessment; 6. Those who have a history of lumbar spine surgery, spinal canal tumors, or neurological diseases.
