基于健康信念模型为基础，构建风险沟通方案，探讨风险沟通对脑卒中患者跌倒恐惧的干预效果，验证该方案对改善脑卒中患者跌倒恐惧的有效性和可行性。  

Objectives of Study：

aking the Health Belief Model as the theoretical basis, this study developed a risk communication protocol, explored its intervention effects on fear of falling in stroke patients, and verified the effectiveness and feasibility of the protocol for relieving fear of falling among stroke survivors.

纳入标准：

Inclusion criteria

排除标准：

1.合并肌肉骨骼和神经系统疾病（例如骨折、严重骨质疏松症、帕金森病和脊髓损伤）造成的活动障碍； 2.合并严重心、肺、肝、肾等疾病而活动受限； 3.严重失语、认知障碍、精神异常等不能配合者； 4.正在参与可能影响本研究结果评价的其他临床试验者。

Exclusion criteria：

1. Activity limitations caused by combined musculoskeletal and nervous system diseases (such as fractures, severe osteoporosis, Parkinson's disease, and spinal cord injuries); 2. Activity restrictions due to severe heart, lung, liver, kidney, and other diseases; 3. Those who cannot cooperate due to severe aphasia, cognitive impairment, or mental disorders; 4. Participants currently involved in other clinical trials that may affect the evaluation of this study's results.

对照组采用神经内科常规护理，于患者入院当日进行科室环境及制度、辅助设施使用的注意事项、医务人员介绍等宣教，患者入院后至出院前期间进行疾病相关知识、静脉通路护理、并发症预防措施、跌倒危险因素及危害、防跌倒措施、跌倒后处理、用药、饮食、运动等指导。

The control group received routine neurological nursing. On the day of admission, patients were given health education regarding ward environment, hospital regulations, precautions for the use of auxiliary facilities, and introduction of medical staff. From admission to discharge, standardized guidance was continuously provided, including disease-related knowledge, venous access care, complication prevention, risk factors and hazards of falls, fall prevention strategies, post-fall management, medication guidance, diet and exercise instructions.
