研究目的:

基于5G智慧数据终端采集的多时间点数据,采用潜类别增长模型(LCGA)识别COPD住院患者在15天呼吸肌负荷训练联合膈肌排痰干预中的呼吸功能改善轨迹类别,并探索各类别轨迹的预测因素。  

Objectives of Study:

To identify the trajectories of respiratory function improvement in hospitalized COPD patients undergoing a 15-day intervention of respiratory muscle load training combined with diaphragmatic sputum drainage using a 5G smart data terminal, and to explore the predictors of different trajectory classes, based on multi-time-point data collected by the terminal and analyzed with latent class growth analysis (LCGA).

纳入标准:

Inclusion criteria

排除标准:

1.需有创或无创机械通气者; 2.存在气胸、活动性咯血、不稳定心绞痛或急性心肌梗死者; 3.合并严重神经系统疾病(如卒中后吞咽障碍)、肌肉骨骼疾病或恶性肿瘤影响康复实施者; 4.认知障碍或精神疾病无法配合训练者; 5.入院前1个月内参加过其他呼吸康复临床试验者; 6.无法完成至少7天住院康复者。

Exclusion criteria:

1. Patients requiring invasive or non-invasive mechanical ventilation; 2. Presence of pneumothorax, active hemoptysis, unstable angina, or acute myocardial infarction; 3. Coexisting severe neurological diseases (e.g., post‑stroke dysphagia), musculoskeletal disorders, or malignant tumors that interfere with rehabilitation; 4. Cognitive impairment or psychiatric disorders preventing cooperation with training; 5. Participation in other respiratory rehabilitation clinical trials within one month prior to admission; 6. Inability to complete at least 7 days of inpatient rehabilitation.

All patients used the 5G intelligent respiratory rehabilitation terminal and received daily 1-2 sessions of respiratory muscle threshold load training combined with external diaphragm sputum removal intervention during their hospital stay. Each session lasted approximately 30-40 minutes.
