研究目的:

建立基于超声低回声条带特征的隐性唇裂客观诊断体系,制定标准化操作规范,为解决临床漏误诊问题提供循证医学证据与技术支撑。  

Objectives of Study:

To establish an objective diagnostic system for microform cleft lip using ultrasound hypoechoic band features, formulate standardized operating procedures, and provide evidence-based medical evidence and technical support to address clinical missed diagnosis and misdiagnosis.

纳入标准:

Inclusion criteria

排除标准:

病例组排除标准: (1)患侧已行手术或其他治疗。 (2) 唇鼻区域既往外伤史。 (3) 除上述异常外有其他影响唇、鼻形态或功能的畸形。 (4) 唇鼻区域 1 个月内存在活动性感染,如痤疮、疱疹、毛囊炎、湿疹等,或 1个月内有局部皮肤治疗史。 (5)无法配合完成上唇超声检查。

Exclusion criteria:

Exclusion Criteria for Case Group: (1) Prior surgery or other treatment on the affected side. (2) Previous history of trauma to the lip and nose area. (3) Presence of other deformities affecting the morphology or function of the lip or nose, beyond the aforementioned abnormalities. (4) Active infection in the lip and nose area within the past 1 month, such as acne, herpes, folliculitis, eczema, etc., or a history of local dermatological treatment within the past 1 month. (5) Inability to cooperate in completing the upper lip ultrasound examination.

目前隐性唇裂的临床诊断完全依靠典型的临床表现,包括患侧唇峰不连续、唇峰上抬、红唇粘膜切迹、白唇皮肤凹沟、白唇色沉、双侧鼻孔不对称等。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Currently, the clinical diagnosis of microform cleft lip relies entirely on typical clinical manifestations, including discontinuity of the Cupid's bow on the affected side, elevation of the Cupid's bow, notching of the vermilion mucosa, grooving of the philtral skin, hyperpigmentation of the philtral skin, and asymmetry of the bilateral nostrils, etc.

利用高频超声对所有受试者进行上唇软组织检测,记录是否存在上唇软组织内的低回声条带,以及条带的宽度、位置、随表情的动态变化等。把专家组临床一致诊断为隐性唇裂作为诊断的金标准,计算超声发现上唇低回声条带对于隐性唇裂的诊断效能。

Index test:

High-frequency ultrasound was used to examine the upper lip soft tissue in all subjects, recording the presence or absence of hypoechoic bands, their width, location, and dynamic changes with facial expressions. The clinical consensus diagnosis of microform cleft lip, made by an expert panel, served as the gold standard to assess the diagnostic efficacy of ultrasound-detected hypoechoic bands in the upper lip for microform cleft lip.

Target condition (The target condition is a particular disease or disease stage that the index test will be intended to identify. Please specify the characteristics in detail; the population should has a complete spectrum and good representative):

The target population for this study is individuals with clinically suspected microform cleft lip. Characteristics include the presence of one or more typical external manifestations of microform cleft lip, such as discontinuity of the Cupid's bow, elevation of the Cupid's bow, notching of the vermilion mucosa, grooving of the philtral skin, hyperpigmentation of the philtral skin, and asymmetry of the bilateral nostrils. To ensure good representativeness, the target population should include a consecutive series of cases covering different ages (including infants, children, and adults), sexes, and varying degrees of phenotypic presentation, reflecting the real-world disease spectrum and avoiding selection bias from including only typical or atypical cases.

容易混淆的疾病人群(即与目标疾病不易区分的一种或多种不同疾病,应避免采用正常人群对照的病例-对照设计):

本研究旨在解决隐性唇裂的新生儿期漏诊问题(黄金治疗时间3-12月龄),因隐性唇裂在新生儿期外观极为轻微,常被误认为是正常唇鼻形态,但漏诊的影响可持续至年长儿及成人。因此,最需要鉴别的是外观正常的健康人群,而非其他有明显病史的疾病。为增强代表性,本研究入组病例无年龄限制(涵盖新生儿至成人),但核心诊断任务始终是区分“正常”与“患病”。正常人群作为对照组直接回应了这一需求。

Population with condition difficult to distinguish from the target condition, the normal population in a case-control study design should be avoid:

This study aims to address the problem of neonatal missed diagnosis of microform cleft lip, whose golden treatment window is between 3 and 12 months of age. Due to the extremely subtle external appearance of microform cleft lip during the neonatal period, it is often mistaken for normal lip and nasal morphology. However, the impact of missed diagnosis can persist into older children and even adulthood. Therefore, the most critical differential diagnosis is distinguishing the condition from healthy individuals with normal appearance, rather than from other diseases with clear medical histories. To enhance representativeness, this study imposes no age restrictions on enrolled cases (covering neonates to adults), while the core diagnostic task remains to differentiate between "normal" and "diseased." The use of a healthy normal control group directly addresses this need.
