开展13N-氨水PET诊断、评估心肌缺血新技术。

Objectives of Study：

Developing a new technique for the diagnosis and assessment of myocardial ischaemia using 13N-ammonia PET.

药物成份或治疗方案详述：

注射555-925 MBq（15-25 mCi）13N-氨水后4-5分钟开始图像采集。静息及负荷图像总共采集10-15分钟，负荷和静息显像的时间间隔通常在40到50分钟。

Description for medicine or protocol of treatment in detail：

Imaging should commence 4–5 minutes after the injection of 555–925 MBq (15–25 mCi) of 13N-ammonium hydroxide. Rest and stress images should be acquired over a total of 10–15 minutes, with the interval between stress and rest imaging typically being 40 to 50 minutes.

Inclusion criteria

排除标准：

1. 不稳定型心绞痛、急性冠脉综合征、急性心肌梗死后小于2至4天； 2. 窦房结病变，如病窦综合征或症状性心动过缓，并无正在工作的起搏器； 3. 二度或三度房室传导阻滞又无功能性起搏器； 4. 收缩压低于90 mmHg。对具有自主神经功能障碍、低血容量、左冠状动脉主干狭窄、狭窄的瓣膜性心脏病、心包炎、心包积液，或脑血管功能不全的颈动脉狭窄疾病患者而言，严重低血压所带来的风险更高； 5. 未加以控制的高血压（收缩压>200 mmHg 或舒张压>110 mmHg）； 6. 持续喘息并伴有支气管痉挛肺部疾病的患者或有显著反应性气道病史的患者不应该接受腺苷负荷试验； 7. 已知对腺苷过敏； 8. 妊娠或哺乳期女性。

Exclusion criteria：

1. Unstable angina, acute coronary syndrome, or within 2 to 4 days following an acute myocardial infarction; 2. Sick sinus syndrome or symptomatic bradycardia, without an active pacemaker; 3. Second- or third-degree atrioventricular block without a functional pacemaker; 4. Systolic blood pressure below 90 mmHg. The risk associated with severe hypotension is higher in patients with autonomic dysfunction, hypovolaemia, stenosis of the left main coronary artery, valvular heart disease with stenosis, pericarditis, pericardial effusion, or carotid artery stenosis associated with cerebrovascular insufficiency; 5. Uncontrolled hypertension (systolic blood pressure >200 mmHg or diastolic blood pressure >110 mmHg); 6. Patients with persistent wheezing and bronchospastic lung disease, or those with a history of significant reactive airway disease, should not undergo an adenosine stress test; 7. Known hypersensitivity to adenosine; 8. Pregnant or breastfeeding women.
