本研究构建基于行为改变论（behaviour change wheel，BCW）理论的老年共病患者出院准备服务方案，验证该方案对老年共病患者出院准备度、出院准备质量及再入院率的影响，以期为临床构建出院准备干预方案提供参考。

It evaluated the impact of this program on discharge readiness, discharge planning quality, and readmission rates among these patients, aiming to supply reference for clinical development of discharge planning strategies.

1.患者有意识障碍或既往有精神病史。
