研究目的：

本研究基于床旁彩色多普勒超声联合血浆D-二聚体（迈瑞CX-9000，含年龄校正截断值）检测数据， 1. 评估常规固定截断值、年龄校正截断值下D-二聚体单独检测、床旁超声单独检测及二者联合检测在ICU患者下肢深静脉血栓（DVT）中的整体诊断效能； 2. 明确床旁超声联合年龄校正D-二聚体对ICU患者孤立性远端血栓（胫后静脉、腓静脉等）的真实检出情况，探讨床旁彩色多普勒超声在远端小血栓筛查中的核心价值与应用策略； 3. 为优化我院ICU DVT的标准化筛查与诊断流程提供科学、可行的实践依据。

Objectives of Study：

This study is based on data from bedside color Doppler ultrasound combined with plasma D-dimer (Mindray CX-9000, including age-adjusted cutoff values) testing. The objectives are: 1. To evaluate the overall diagnostic efficacy of D-dimer testing alone using conventional fixed cutoff values and age-adjusted cutoff values, bedside ultrasound alone, and their combination in diagnosing lower extremity deep vein thrombosis (DVT) in ICU patients; 2. To determine the actual detection rate of isolated distal thrombosis (such as posterior tibial and peroneal veins) in ICU patients using bedside ultrasound combined with age-adjusted D-dimer, and to explore the core value and application strategy of bedside color Doppler ultrasound in screening small distal thrombi; 3. To provide a scientific and feasible practical basis for optimizing the standardized screening and diagnostic process for DVT in the ICU of our hospital.

Inclusion criteria

排除标准：

1.既往有下肢DVT或肺栓塞病史（可能影响检测结果解读）；
2.入ICU前已接受抗凝治疗（如低分子肝素、华法林）；
3.合并严重肝肾功能衰竭（肝功能Child-Pugh C级或血肌酐>442 μmol/L，可能干扰D-二聚体代谢）；
4.超声检查仅完成近端/远端单一部位评估，资料不完整者；
5.临床随访资料缺失，无法完成综合评估者；

Exclusion criteria：

1. History of lower extremity DVT or pulmonary embolism (which may affect the interpretation of test results); 2. Received anticoagulant therapy (e.g., low molecular weight heparin, warfarin) before ICU admission; 3. Concomitant severe liver or kidney failure (Child-Pugh Class C liver function or serum creatinine >442 μmol/L, which may interfere with D-dimer metabolism); 4. Incomplete ultrasound examination with assessment of only proximal or distal sites; 5. Missing clinical follow-up data, precluding comprehensive evaluation.

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):	In this study, bedside color Doppler ultrasound combined with comprehensive clinical assessment (including underlying diseases, DVT risk factors, clinical signs and symptoms, dynamic laboratory test results, and subsequent clinical follow-up outcomes) was used as the diagnostic reference standard.
