研究目的：

本研究将通过一款名为H-DAO的自主研发小程序和微信群聊进行个人健康行为识别和社区互动，并赋予虚拟社区中的虚拟物品“物质性”属性，通过增加人与人之间的合作竞争互动，使原本“任务性”的自我健康管理“复杂化”“社会化”，建立一种更贴近现实生活的虚拟社区自我健康管理模式。以此作为干预手段，开展随机对照试验，研究现实社会的健康行为和虚拟社区行为、健康价值及其附加价值、虚拟与现实社区交互三个方面之间的转化机制和基于虚拟社群的心脏康复自我健康管理模式对于患者康复效果和依从性促进问题。  

Objectives of Study：

This study will use a self-developed mini-program named H-DAO and group chats on WeChat to identify personal health behaviors and facilitate community interaction. Virtual items within the virtual community will be given "materiality" attributes. By increasing cooperative and competitive interactions among individuals, the originally "task-oriented" self-health management will be "socialized" and made more "complex." This approach will establish a virtual community-based self-health management model that more closely resembles real-life dynamics. This model will serve as an intervention in a randomized controlled trial. The study will investigate the mechanisms of transformation between real-life health behaviors and behaviors within the virtual community, health values and their added value, and the interaction between virtual and real communities. In addition, it will explore the effects of this virtual community-based cardiac rehabilitation self-health management model on patient rehabilitation outcomes and adherence promotion.

纳入标准：

Inclusion criteria

排除标准：

1.运动康复的禁忌证（例如，未经治疗的室性心动过速、严重的心力衰竭、无法控制的高血压或低血压、显著的运动限制等）； 2.患者经指令后无法使用微信小程序； 3.需要助行器移动的患者； 4.参加其他临床试验的患者； 5.其他原因，医生认为其不适合参加本次研究。

Exclusion criteria：

1. Contraindications to exercise rehabilitation (e.g., untreated ventricular tachycardia, severe heart failure, uncontrolled hypertension or hypotension, significant movement limitations, etc.); 2. Unable to use the WeChat mini-program after receiving instructions; 3. Require a walking aid for mobility; 4. Currently participating in other clinical trials; 5. Any other reasons for which the physician deems the patient unsuitable for participation in this study.

医护人员会根据患者的身体情况和历史微信步数，告知每日或每周的的安全锻炼步数和安全运动建议。患者需要每日按照小程序使用说明进行操作，操作包括但不限于查看每日和历史步数，运动打卡、吃药打卡，健康知识学习、收集虚拟徽章，积分兑换礼品等。

The medical staff will provide the patients with the safe exercise steps and safety exercise suggestions for daily or weekly basis, based on the patients' physical conditions and their historical step counts on WeChat. The patients need to follow the operation instructions of the mini-program every day, which includes but is not limited to checking the daily and historical step counts, logging exercise, taking medicine, learning health knowledge, collecting virtual badges, exchanging points for gifts, etc.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

由于电子问卷，填写即录入，研究者根据受试者的信息填写，进行检查，如有异常答案，进行疑问发放。经过审核、签字后的调查表应及时送交项目负责人。当所有受试者结果数据经输入并核对无误后，由研究者写出数据库检查报告，其内容包括研究完成情况、入选/排除标准检查、完整性检查、逻辑一致性检查等。 如涉及医院病历数据将严格遵守《中华人民共和国个人信息保护法》《中华人民共和国民法典》《涉及人的生物医学研究伦理审查办法》《医疗机构管理条例》《医疗事故处理条例》《医疗机构病历管理规定》等相关法律法规，并经过相关部门审批，以及保密协议签订。 此项研究将对所有患者的数据保密，小程序后台信息将储存在有专业人员和防火墙保护的数据中心，数据将以代码编号进行标识。除项目负责人外，其他人员无法识别任何参与者身份信息。最后，本研究问卷调查等资料将按照相关规定在北京大学健康医疗大数据研究院保管3年，以备查验，之后可识别个人信息的部分将被销毁，确保隐私保护。必要时，政府或伦理委员会可以查阅资料，但不对外披露。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Due to the electronic questionnaire, the information is filled in and entered immediately. The researchers fill in the information based on the participants' details and conduct checks. If there are any abnormal answers, questions will be issued. After review and signature, the survey forms should be promptly submitted to the project leader. When all the results data of the participants have been input and verified without errors, the researcher should write a database inspection report. The content of the report includes the completion status of the research, the inspection of inclusion/exclusion criteria, the completeness check, the logical consistency check, etc. If hospital medical record data is involved, it will strictly comply with relevant laws and regulations such as the "Personal Information Protection Law of the People's Republic of China", the "Civil Code of the People's Republic of China", the "Ethical Review Measures for Human Biomedical Research", the "Regulations on the Management of Medical Institutions", the "Regulations on the Handling of Medical Accidents", and the "Regulations on the Management of Medical Records of Medical Institutions", and will be approved by relevant departments and a confidentiality agreement will be signed. This research will keep all patients' data confidential. The information in the background of the mini-program will be stored in a data center with professional personnel and firewalls for protection. The data will be identified by code numbers. Except for the project leader, no other personnel can identify the identity information of any participant. Finally, the research questionnaire survey and other materials will be stored at the Beijing University Institute of Health Medical Big Data for 3 years in accordance with relevant regulations for inspection. The parts that can identify personal information will be destroyed after that to ensure privacy protection. If necessary, the government or the ethics committee can review the materials, but they will not be disclosed to the public.
