1.分析冠心病患者医院-家庭过渡期发生用药偏差的危险因素； 2.构建并验证基于机器学习的冠心病患者医院-家庭过渡期用药偏差风险预测模型，并选出最优模型。  

Objectives of Study：

1.To analyze the risk factors for medication discrepancies in patients with coronary heart disease during the hospital-to-home transition period. 2.To develop and validate machine learning–based risk prediction models for medication discrepancies in patients with coronary heart disease during the hospital-to-home transition period, and to select the optimal model among them.

Inclusion criteria

排除标准：

1.存在认知、语言、听觉或视觉障碍，无法进行有效的沟通和交流； 2.伴有严重的器质性疾病。

Exclusion criteria：

1. Presence of cognitive, language, hearing, or visual impairments that prevent effective communication and interaction; 2. Concomitant severe organic diseases.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究采用病历报告表和医院信息系统采集数据，同时通过患者及主要照顾者填写相关量表进行补充； 数据管理采用双人核对的方式录入Excel进行数据管理，基于 SPSS 27.0、R 4.2.3 和 Python软件进行数据处理与统计分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data were collected using case report forms and the hospital information system, and supplemented with relevant scales filled out by patients and their primary caregivers. Data management was performed by double-entry verification into Excel. Statistical analysis and data processing were conducted based on SPSS 27.0, R 4.2.3, and Python software.
