研究目的：

主要研究目标：探讨HAIC为基础联合系统药物治疗作为不可切除BTC初始降阶梯治疗方案，序贯根治性/潜在根治性治疗方式在提升不可切除BTC患者OS和临床完全缓解（clinical complete response，cCR）率的价值。 次要研究目标：根据进展期BTC的分期特征探索HAIC为基础联合系统治疗作为初始降阶梯治疗方案的价值。探讨具有潜在根治性治疗效果的不同局部治疗方式的最佳适应患者特征；ORR、PFS、不良反应；探索新型影像组学和液态活检标记物对于cCR的诊断价值。  

Objectives of Study：

Primary objective: To investigate the value of HAIC combined with systemic therapy as de-escalation therapy followed by radical/potentially radical treatment in improving overall survival (OS) and the rate of clinical complete response (cCR) for patients with unresectable BTC.Secondary study objectives: To explore the value of HAIC combined with systemic therapy as de-escalation therapy based on the clinical stage for patients with advanced BTC. To investigate the optimal patients for different local treatment modalities with potentially curative effects; ORR, PFS, and adverse events; and to explore the diagnostic value of novel radiomics and liquid biopsy markers for cCR.

纳入标准：

Inclusion criteria

排除标准：

1.双原发或多原发癌 2.胆总管下段癌 3.造影剂、奥沙利铂过敏 4.怀孕期或哺乳期妇女 5.肝外多处转移或合并恶性胸腹水 6.有器官移植史 7.伴有感染需要抗感染治疗者 8.严重且不可纠正的凝血功能障碍

Exclusion criteria：

1. Dual primary or multiple primary cancers 2. Cancer in the lower segment of the bile duct 3. Allergy to contrast agent or oxaliplatin 4. Pregnant or lactating women 5. Multiple extrarenal metastases or combined with malignant pleural or peritoneal effusion 6. History of organ transplantation 7. Patients with infection requiring anti-infection treatment 8. Severe and irreparable coagulation dysfunction
