Objectives of Study：

Perioperative Inspiratory Muscle Training Combined with Optimal Intraoperative PEEP: Effect on Perioperative Neurocognitive Disorders in Elderly Hip Fracture Patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1.术前存在神经系统疾病（脑卒中、癫痫、颅脑外伤、脑肿瘤等）或精神疾病史；
2.术前中度以上认知障碍、痴呆、抑郁或谵妄；
3.术前严重心肺功能不全，不能耐受PEEP或呼吸训练；
4.ASA≥Ⅳ级，无法耐受麻醉手术；
5.术前合并感染或全身炎症状态（体温>38℃，CRP>20 mg/L）；
6.术前7天内使用镇静、抗精神病或抗胆碱药物；

Exclusion criteria：

1.Preoperative history of neurological diseases (e.g., stroke, epilepsy, craniocerebral trauma, brain tumor) or mental illness;
2.Preoperative moderate to severe cognitive impairment, dementia, depression, or delirium;
3.Preoperative severe cardiopulmonary insufficiency unable to tolerate PEEP (Positive End-Expiratory Pressure) or breathing training;
4.ASA physical status classification ≥ Ⅳ, unable to tolerate anesthesia and surgery;
5.Preoperative infection or systemic inflammatory state (body temperature > 38℃, CRP > 20 mg/L);
6.Use of sedatives, antipsychotics, or anticholinergic drugs within 7 days before surgery.

盲法：

呼吸训练过程中尽量采用标准化语，避免向患者透露干预组别。 PEEP 组患者术中设置与标准通参数一致，不主动告知患者其 PEEP 设定。术后认知功能评估由独⽴评估者完成，评估者不参与干预与术中管理，对分组情况不知情。认知量表（如 MMSE 评分）使统培训流程，评估员经培训合格后上岗。数据由第方独立统计员进行处理，分析过程中以编码式处理组别信息直分析完成予解盲。

Blinding：

During the respiratory training process, standardized language should be used as much as possible, and patients should not be informed of their intervention group. For patients in the PEEP group, intraoperative settings are consistent with standard ventilation parameters, and patients are not actively told their PEEP settings. Postoperative cognitive function assessments are conducted by independent evaluators who do not participate in interventions or intraoperative management and are unaware of group assignments. Cognitive scales (such as MMSE scores) are used following standardized training procedures, and evaluators start work only after being trained and certified. Data are processed by independent statisticians, and during analysis, group information is handled in a coded manner; unblinding occurs only after analysis is completed.
