研究目的：

脊柱内镜下腰椎间盘切除术后的再次手术主要归因于纤维环缺损处的髓核再突出。常规靶点减压术虽能有效解除神经压迫，但椎间盘内减压的方式和程度尚无统一标准，术后仍有相当比例的患者因同节段再突出需要再次手术。 本研究旨在评估一项创新的辅助技术——“盘内环形减压术”的有效性与安全性。该技术在完成常规硬膜外髓核摘除后实施，其核心是通过盘内360°环形减压操作，进一步摘除椎间盘内残留和退变的髓核组织，同时降低椎间盘内张力，从而降低术后同节段再手术率。  

Objectives of Study：

Re-operations after lumbar discectomy under spinal endoscopy are primarily attributed to the re-protrusion of the nucleus pulposus at the defect site of the annulus fibrosus. Although conventional target decompression can effectively relieve nerve compression, there is no unified standard for the method and degree of decompression within the intervertebral disc. A considerable proportion of patients still require re-operations due to re-protrusion at the same level after surgery.This study aims to evaluate the efficacy and safety of an innovative auxiliary technique, "intra-disc circular decompression," which is performed after conventional epidural nucleus pulposus removal. The core of this technique involves a 360° circular decompression procedure within the disc, aimed at further removing residual and degenerated nucleus pulposus tissue from the intervertebral disc, while reducing the internal tension of the disc, thereby decreasing the rate of reoperation at the same level after surgery.

纳入标准：

Inclusion criteria

排除标准：

1.腰椎多节段椎间盘突出；
2.椎间盘脱出游离至远端（超出椎间隙上下缘5mm）；
3.伴椎间盘钙化；
4.中度以上椎管狭窄（硬膜囊面积<100mm²）；
5.既往有腰椎手术史；
6.存在腰椎不稳或者滑脱（动力位X线椎体滑移>3mm或椎间角度变化>15°）或腰椎滑脱（≥Ⅰ度）；
7.合并腰椎感染或者肿瘤；
8.合并严重内科疾病无法耐受手术；
9.临床资料不完整或失访；

Exclusion criteria：

1.Multi-level lumbar disc herniation; 2.Disc herniation with free disc fragments at the distal end(extending beyond the upper and lower edges of the intervertebral space by 5mm); 3.Associated disc calcification; 4.Moderate or severe spinal stenosis; 5.Previous history of lumbar surgery; 6.Presence of lumbar instability or spondylolisthesis (dynamic X-ray vertebral slippage >3mm or intervertebral angle change >15°) or lumbar spondylolisthesis (>=Grade I); 7.Concurrent lumbar infection or tumor; 8.Patients with severe internal diseases that are intolerable to surgery; 9.Incomplete clinical data or loss to follow-up;
