Objectives of Study：

To explore the emergency nursing plan and application effect of intravenous thrombolysis in patients with acute ischemic wake-up stroke.

纳入标准：

Inclusion criteria

排除标准：

1. 影像学证实为颅内出血或非缺血性卒中（如肿瘤、癫痫）。 2. 严重肝肾功能不全或凝血功能障碍（INR>1.7，血小板<100×10^9/L）。 3. 既往存在严重残疾（mRS>=3分）。

Exclusion criteria：

1. Imaging confirmed intracranial hemorrhage or non-ischemic stroke (such as tumor, epilepsy). 2. Severe liver and kidney dysfunction or coagulation dysfunction (INR > 1.7, platelet < 100 × 10^9/L). 3. Previous severe disability (mRS >= 3).

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集 （1）数据来源：通过徐州医科大学附属医院急诊扁鹊系统、急救智慧云平台采集急性脑卒中患者就诊信息。 （2）数据类型：数据类别包括人口学信息、疾病诊断、就诊时间、用药时间等）。 数据范围：应用2025年7月15日至2027年7月15日在急诊抢救室就诊的急性脑卒中患者信息，样本量约200例、入选标准为研究病例均符合醒后卒中静脉溶栓纳入标准及排除标准。 数据管理： （1）数据安全与隐私保护措施 匿名化/去标识化：在数据分析过程中使用EXCEL 表登记患者信息，查看文件时将姓名模糊处理，分析时仅有编号，不涉及患者姓名。 访问控制：数据存储于急诊医学科、仅限研究者查看使用、在使用前会检查数据完整性。 伦理合规：研究为队列研究，且不涉及信息传输，保证就诊患者信息安全，符合伦理规定。 （2）数据质量评估 完整性检查：在研究中若出现患者信息缺失关键数据，则给予删除。 逻辑一致性：研究中会通过EXCEL表验证数据合理性，如果发病时间、就诊时间出现负值，或者就诊时间早于发病时间，则不符合逻辑。 重复数据筛查：两位研究者同步录入及核对数据，以确保识别并处理重复记录。 （3）风险分析与应对 隐私泄露风险：急诊抢救室脑卒中患者就诊数量比较大，匿名化后数据被重新识别的可能性基本不存在。 数据使用合规性：研究者使用脑卒中患者就诊发病时间，CT检查时间、医患沟通时间。溶栓时间等数据，不会影响患者的预后及个人权益，符合医院数据使用规定。 应急预案：若研究中有数据丢失或泄露应急预案，若匿名化符合标准（无法重新识别个人），通常无需报告，但需要记录事件。如果是含有患者信息数据应急预案，做好预防措施，做好数据备份。根据最小化原则设置访问控制权限，同时加密与匿名化。同时定期培训，签署保密协议。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection ( 1 ) Data source : Through the emergency magpie system and emergency wisdom cloud platform of the Affiliated Hospital of Xuzhou Medical University, the medical information of patients with acute stroke was collected. ( 2 ) Data types : Data categories include demographic information, disease diagnosis, treatment time, medication time, etc. ). Data range : The information of patients with acute stroke who were admitted to the emergency room from July 15,2025 to July 15,2027 was applied. The sample size was about 200 cases. The inclusion criteria were that the study cases met the inclusion criteria and exclusion criteria for intravenous thrombolysis in wake-up stroke. Data management : ( 1 ) Data security and privacy protection measures Anonymization / de-identification : In the process of data analysis, EXCEL table is used to register patient information, and the name is blurred when viewing the file. Only the number is numbered during the analysis, and the name of the patient is not involved. Access control : data is stored in the emergency medicine department, only for researchers to view and use, and data integrity is checked before use. Ethical compliance : the study is a cohort study, and does not involve information transmission, to ensure the safety of patient information, in line with ethical requirements. ( 2 ) Data quality assessment Integrity check : If the patient information is missing in the study, the key data is deleted. Logic consistency : In the study, the rationality of the data will be verified by EXCEL table. If the onset time and treatment time are negative, or the treatment time is earlier than the onset time, it is not logical. Duplicate data screening : The two researchers simultaneously entered and checked the data to ensure that duplicate records were identified and processed. ( 3 ) Risk analysis and response Risk of privacy leakage : The number of stroke patients in the emergency room is relatively large, and the possibility of data being re-identified after anonymization basically does not exist. Data use compliance : The researchers used the onset time of stroke patients, CT examination time, and doctor-patient communication time. Thrombolysis time and other data will not affect the prognosis of patients and personal rights and interests, in line with hospital data use regulations. Emergency plan : If there is data loss or leakage in the study, if the anonymization meets the standard ( unable to re-identify individuals ), there is usually no need to report, but the event needs to be recorded. If it is an emergency plan containing patient information data, take preventive measures and do a good job of data backup. The access control permissions are set according to the minimization principle, and encryption and anonymization are performed simultaneously. At the same time, regular training, signed a confidentiality agreement.
