研究目的:

本研究拟开展前瞻性临床研究,系统评估尿液HK2在非肌层浸润性膀胱癌(NMIBC)术后复发监测中的阴性预测值,构建基于HK2的“阴性排除”决策模型,优化膀胱镜检查随访策略;同时结合组织多重免疫荧光与尿液外泌体分析,明确HK2的细胞来源及代谢学基础,为NMIBC术后无创复发监测提供科学依据,减少不必要的侵入性检查,提高患者依从性并降低医疗负担。  

Objectives of Study:

This prospective clinical study aims to systematically evaluate the negative predictive value of urinary HK2 in monitoring postoperative recurrence of non-muscle invasive bladder cancer (NMIBC), and to construct a HK2-based "negative exclusion" decision model for optimizing cystoscopy follow-up strategies. Furthermore, we will clarify the cellular origin and metabolic basis of HK2 through multiplex immunofluorescence and urinary exosome analysis, to provide a scientific basis for non-invasive postoperative recurrence monitoring of NMIBC, reduce unnecessary invasive examinations, improve patient compliance, and alleviate medical burden.

纳入标准:

Inclusion criteria

排除标准:

1. 合并上尿路肿瘤、活动性泌尿系感染或严重代谢性疾病者;
2. 正在接受全身性抗肿瘤治疗(包括系统静脉化疗、靶向治疗或免疫检查点抑制剂)者或近期参与其他干预性研究影响尿液标志物检测结果者;
3. 合并严重心、肝、肾功能不全者。

Exclusion criteria:

1. Patients with concurrent upper urinary tract tumor, active urinary tract infection, or severe metabolic diseases;
2. Those receiving systemic anti-tumor therapy (including systemic intravenous chemotherapy, targeted therapy, or immune checkpoint inhibitors) or those who have participated in other interventional studies recently that may interfere with the detection of urinary biomarkers;
3. Patients with severe complications involving cardiac, hepatic, or renal insufficiency.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Paper-based standardized Case Record Forms (CRF) will be used to collect patients' baseline clinical data, follow-up information, and urine HK2 test data. Researchers will double-enter the data into Excel spreadsheets for management, and the principal investigator will conduct regular data verification and backup to ensure data integrity and accuracy.
