研究目的:

探讨外骨骼机器人辅助步态训练对下肢烧伤患者瘢痕生物力学特征的影响,尝试在烧伤患者的功能评估中加入生物力学特征的分析,通过相关性研究证明外骨骼机器人辅助步态训练可以对瘢痕生物力学特征产生影响,瘢痕生物力学特征亦可作为一个常规的评估指标指导日常的治疗工作,为日后针对瘢痕组织生物力学特征的临床研究打下理论基础。  

Objectives of Study:

This study explores the impact of exoskeleton-assisted gait training on the biomechanical characteristics of scars in patients with lower limb burns. It attempts to incorporate the analysis of biomechanical characteristics into the functional evaluation of burn patients and aims to demonstrate through correlation studies that exoskeleton-assisted gait training can affect scar biomechanical characteristics. These scar biomechanical characteristics can also serve as a routine evaluation indicator to guide daily treatment work, laying a theoretical foundation for future clinical research on the biomechanical characteristics of scar tissue.

纳入标准:

Inclusion criteria

排除标准:

①病情导致功能障碍非常严重;②患有骨代谢疾病(糖尿病、甲亢等);③合并其它骨骼、肌肉、关节等病变足以影响患者进行外骨骼机器人辅助步态训练的相关疾病;④哺乳或妊娠的妇女;⑤心脏病合并房颤的患者;⑥血压过高者(>180/120mmHg)不适宜进行外骨骼机器人辅助步态训练及康复治疗的患者;⑦合有肝、肾、造血系统和内分泌系统等严重原发性疾病、精神病患者;⑧无法理解或完成整个实验流程的患者。

Exclusion criteria:

1. Patients with severe functional impairment resulting from their medical condition; 2. Patients with bone metabolic disorders (e.g., diabetes, hyperthyroidism); 3. Patients with concurrent pathologies of the bones, muscles, joints, or other systems that are sufficient to interfere with exoskeleton-assisted gait training; 4. Lactating or pregnant women; 5. Patients with cardiac disease complicated by atrial fibrillation; 6. Patients with severely elevated blood pressure (> 180/120 mmHg) for whom exoskeleton-assisted gait training and rehabilitation therapy are deemed unsuitable; 7. Patients with severe primary diseases affecting the liver, kidneys, hematopoietic system, or endocrine system, as well as patients with psychiatric disorders; 8. Patients unable to comprehend or complete the entire experimental protocol.

盲法:

本研究采用单盲设计。 施盲对象为结果评估者(或数据测量人员)。 由于本研究干预措施涉及外骨骼机器人辅助步态训练与常规步行训练,患者及实施治疗的治疗师在治疗过程中必然知晓具体的分组情况,无法实施盲法。因此,为了确保评估结果的客观性,本研究仅对负责进行生物力学指标、平衡功能、步行功能及运动耐量等结局指标测量与评估的人员实施盲法,使其在评估过程中不知晓患者的具体分组,从而减少测量偏倚。

Blinding:

This study utilized a single-blind design. The subjects of blinding were the outcome assessors (or data measurement personnel). Due to the nature of the interventions involving exoskeleton robot-assisted gait training versus conventional gait training, it was impossible to blind the patients and the treating therapists to the group allocation during the treatment process. Therefore, to ensure the objectivity of the evaluation results, blinding was implemented only for the personnel responsible for measuring and assessing outcome measures—such as biomechanical indicators, balance function, walking ability, and exercise tolerance. This ensured that the assessors remained unaware of the specific group assignments during the evaluation process, thereby minimizing measurement bias.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

本研究的数据采集和管理由病例记录表(Case Record Form, CRF)与电子数据采集和管理系统(Electronic Data Capture, EDC)两部分组成,以确保数据的真实性、完整性和安全性。 病例记录表(CRF) 根据研究方案预先设计的标准化CRF将被用于原始数据的记录。CRF的内容涵盖了受试者的基本信息、人口学特征、烧伤病史、基线评估数据、干预过程记录以及治疗后的各项结局指标(包括瘢痕生物力学指标、平衡功能、步行功能及运动耐量等)和不良事件记录。所有CRF均由经过统一培训的研究人员填写,填写时需确保字迹清晰、数据准确。一旦数据录入完成,CRF将由研究负责人审核并签字确认,作为原始档案妥善保存。 电子数据采集和管理系统(EDC) 本研究采用基于互联网的ResMan电子数据采集系统进行数据管理。研究人员将纸质CRF中的数据及时、准确地双录入至ResMan系统中。该系统具备严格的用户权限管理功能,不同角色(如研究者、监查员、数据管理员)仅拥有相应的操作权限,以确保数据的安全性。系统内置的逻辑核查功能将在数据录入时自动检测异常值和逻辑错误,并发出疑问。此外,EDC系统支持实时数据监控、审计追踪及数据备份,能够有效提高数据管理效率,保证研究数据的质量和可溯源性。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection and management for this study consist of two components: the Case Report Form (CRF) and the Electronic Data Capture (EDC) system, ensuring the authenticity, integrity, and security of the data. Case Report Form (CRF) A standardized CRF, designed according to the study protocol, was used for the recording of original data. The CRF covered the subjects' basic information, demographic characteristics, burn history, baseline assessment data, intervention process records, and post-treatment outcome measures (including scar biomechanical indicators, balance function, walking ability, and exercise tolerance) as well as adverse event records. All CRFs were completed by researchers who underwent unified training to ensure clear handwriting and accurate data. Once data entry was completed, the CRFs were reviewed and signed by the principal investigator, then securely archived as source documents. Electronic Data Capture and Management System (EDC) This study utilized the internet-based ResMan electronic data capture system for data management. Researchers transcribed data from the paper CRFs into the ResMan system in a timely and accurate manner, ideally using double data entry. The system features strict user rights management, where different roles (e.g., investigators, monitors, data managers) possess only corresponding access privileges to ensure data security. The built-in logical verification function automatically detected outliers and logical errors during data entry and generated queries. Furthermore, the EDC system supported real-time data monitoring, audit trails, and data backup, effectively improving data management efficiency and ensuring the quality and traceability of the study data.
