研究目的：

本研究从肠道菌群的角度，结合粪便及血清代谢组学，16sRNA和二代基因测序分析，进一步全面探讨气滞血瘀型脓毒症老年患者肠道微生物群及其代谢组学的变化，评估肠道菌群与中医病证及患者预后之间的相关性。并观察此类患者接受耳穴压豆干预时肠道菌群及代谢的变化情况。我们希望获得良好的生物学靶点，寻找潜在益生元，并期望为中医在其中的治疗提供方向。

Objectives of Study：

The study aims to comprehensively investigate alterations in the gut microbiota and metabolomics of elderly sepsis patients with Qi stagnation and blood stasis syndrome, through the integration of fecal and serum metabolomics, 16s rRNA sequencing, and next-generation gene sequencing analysis from the perspective of gut microbiota. It will assess the correlation between intestinal flora and traditional Chinese medicine syndromes as well as patient prognosis. Furthermore, changes in gut microbiota and metabolism will be observed in such patients receiving auricular acupressure bean intervention. We hope to identify promising biological targets, explore potential prebiotics, and provide direction for traditional Chinese medicine in the treatment of this condition.

纳入标准：

Inclusion criteria

1.Age > 65 years and meeting the diagnostic criteria for sepsis.
2.Meeting or not meeting the diagnostic criteria for Acute Gastrointestinal Injury (AGI), diagnosed according to the AGI guidelines proposed by the European Society of Intensive Care Medicine (ESICM) in 2012. Briefly described as follows: AGI Grade I: temporary impairment of gastrointestinal function with spontaneous recovery; AGI Grade II: gastrointestinal dysfunction that does not recover spontaneously but improves with medication or other interventions; AGI Grade III: gastrointestinal dysfunction that cannot be restored by conventional treatment; AGI Grade IV: life-threatening gastrointestinal failure affecting multiple organs.
3.If meeting the diagnostic criteria for AGI, the patient must also meet the criteria for sepsis-associated gastrointestinal dysfunction of the Qi stagnation and blood stasis type, as assessed based on the "Expert Consensus on Integrated Traditional Chinese and Western Medicine for Secondary Sepsis-Related Gastrointestinal Dysfunction" and evaluation by a professional TCM practitioner.

排除标准：

1. 65岁以下患者； 2.孕妇； 3.拒绝签署知情同意书者； 4.无法留取粪便标本患者； 5.入住ICU时有原发性胃肠道损害者，如肠道恶性肿瘤、炎症性肠病等肠道疾病； 6.参加其他试验并中途退出研究的人； 7.预后生存＜72小时。

Exclusion criteria：

1. Patients under 65 years of age; 2. Pregnant women; 3. Patients who refuse to sign the informed consent form; 4. Patients unable to provide stool specimens; 5. Patients with primary gastrointestinal lesions upon ICU admission, such as intestinal malignant tumors, inflammatory bowel disease, or other intestinal disorders; 6. Individuals who have participated in other trials and have withdrawn from the study; 7. Patients with a life expectancy of less than 72 hours.
