研究目的：

高尿酸血症的发生与膳食密切相关，探索患者自主性与医患协作方式相结合的营养管理模式，可显著减少降低血尿酸水平，减少患者靶器官损害，提高生活质量。目前该病营养管理方面还存在患者依从性差、健康管理主要集中在三甲医院，县级及各基层医院管理意识薄弱等问题。中医食养通过辨证施膳与整体食物搭配，能有效改善高尿酸血症患者的尿酸水平及代谢指标，具有实践可行性和辅助治疗价值。智慧医疗催生医疗资源分配与服务模式的革新，为慢病健康管理的发展产生了积极且深远的影响。因此，本科室依托国家区域医疗中心建设平台，作为我省高尿酸及痛风联盟主委单位，利用已经建立的ICCC框架的慢病管理模式，参照我省医改相关政策，以智慧医疗技术为依托，实施智慧营养管理方案，提高患者营养相关知信行水平，使其饮食习惯、生活方式等更趋合理，并最终达到有效地预防高尿酸血症和与之有关的心脑血管疾病。  

Objectives of Study：

Smart healthcare has given rise to innovations in the allocation of medical resources and service models, exerting a positive and far-reaching impact on the development of chronic disease health management. Therefore, our department, relying on the platform of the National Regional Medical Center construction and as the main unit of the provincial hyperuricemia and gout alliance, utilizes the established ICCC framework for chronic disease management, refers to relevant policies of medical reform in our province, and, supported by smart healthcare technology, implements a smart nutrition management plan to enhance patients' knowledge, beliefs, and behaviors related to nutrition, making their dietary habits and lifestyles more reasonable, and ultimately achieving effective prevention of hyperuricemia and related cardiovascular and cerebrovascular diseases.

纳入标准：

Inclusion criteria

排除标准：

1.高尿酸血症患者急性发作或伴有严重心、肝、肾衰竭或恶性肿瘤、重大传染病者； 2.有精神类疾病可正常沟通交流者； 3.正在备孕、哺乳、怀孕者； 4.临床资料不全，中途自行退出研究者； 5.有暴力倾向者。

Exclusion criteria：

1. Patients with acute hyperuricemia or accompanied by severe heart, liver, kidney failure, malignant tumor, or major infectious disease; 2. Those with mental illness who can communicate normally; 3. Those who are preparing for pregnancy, breastfeeding or pregnant; 4. Withdrawing from the study due to incomplete clinical data; 5. People who are violent.
