研究目的:

本研究拟基于基于回顾性临床数据,构建并验证一套集成临床特征、实验室生化指标与术前CT影像组学深层特征的多模态机器学习模型,通过特征选择与集成算法及卷积神经网络提取信息,构建肝移植术后顽固性脾功能亢进的预测模型,发掘其危险因素;并通过前瞻性随访人群评估模型对肝移植术后6个月顽固性脾功能亢进的预测性能,探索其在围手术期决策中的潜在应用价值,以期为顽固性脾亢高危人群的术前识别提供帮助,改善肝移植患者的短期和中长期预后。  

Objectives of Study:

This study aims to develop and validate a multimodal machine learning model based on retrospective clinical data, integrating clinical characteristics, laboratory biochemical indicators, and deep features from preoperative CT radiomics. By employing feature selection, ensemble algorithms, and convolutional neural networks for information extraction, a predictive model for refractory splenomegaly after liver transplantation will be constructed and its associated risk factors identified. The model's predictive performance for refractory splenomegaly at six months post-transplantation will be evaluated using a prospective follow-up cohort, and its potential application value in perioperative decision-making will be explored. This study is expected to facilitate preoperative identification of high-risk patients with refractory splenomegaly and improve short-term and long-term outcomes for liver transplant recipients.

Inclusion criteria

排除标准:

1.移植前已行脾切除术或脾动脉栓塞; 2.术中结扎脾动脉或术后已行脾动脉栓塞; 3.患者已行TIPS等降低门脉压相关手术治疗; 4.围手术期死亡; 5.合并血液系统疾病; 6.其他:如重大伦理或知情同意缺失;

Exclusion criteria:

1. Splenectomy or splenic artery embolization performed before transplantation; 2. Ligation of the splenic artery during surgery or splenic artery embolization performed postoperatively; 3. The patient has undergone TIPS or other surgical treatments to reduce portal pressure; 4. Perioperative death; 5. Complicated with hematologic diseases; 6. Others: such as major ethical issues or lack of informed consent;
