纳入标准：

Inclusion criteria

排除标准：

(1)意识不清者；(2)有视觉、听力、认知功能障碍，不能配合研究者；(3)有精神疾病，有不能进行良好的沟通者；（4）正在参加类似的研究或者参加过类似的研究者

Exclusion criteria：

1. if they were unconscious;
2. had mental disorders;
3. had visual, aural, cognitive, or communicative impairments;
4. had participated in similar studies in the past, or were participating in similar studies at the time of the study.

随机方法（请说明由何人用什么方法产生随机序列）：

根据自愿原则，采用便利抽样方式选取2017年4月至5月入住甘肃省医院并出院的82名2型糖尿病患者及其116名家庭成员。随后根据患者所属社区卫生服务中心（广武门，张掖路和联合新村）进行抽签，分配患者其相关家属属于对照组或实验组。每组选择41名T2D患者和58名相关家庭成员。干预时间是2017年6月至12月。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Based on the principle of voluntariness, 82 T2D patients who had been admitted to Gansu provincial hospital from April to May 2017 and had been discharged from the hospital and their 116 family members were selected using convenience sampling. Random selection was then conducted by casting lots.
