研究目的:

精神分裂症是一种主要的精神疾病,也是致残的主要原因之一,在出现阳性和阴性症状的同时伴随认知功能障碍。寻找精神分裂症客观生物标志物可能为新的治疗策略提供信息,且一直是精神病学研究的主要焦点。 作为一种精确和非侵入性的治疗方法——经颅磁刺激(TMS)已在精神病学研究和治疗中广泛使用,并且其中的θ爆发刺激(TBS,一种特殊形式的模式化rTMS)已被用于调节神经活动、探究脑功能、诊断和治疗等研究和临床目的中。 TMS-EEG是一种能够对大脑振荡活动和脑网络的连接性进行的非侵入性评估和调节方法,可以加深科研及临床人员对精神分裂症人群的大脑动力学和大脑行为关系的研究。 目前尚未有结合TMS-EEG技术研究TBS干预下精神分裂症患者大脑活动的研究,本研究拟从此点出发,从大脑振荡活动、脑功能连接和脑网络层面解释TBS干预治疗精神分裂症的认知控制网络改变的生理学机制。  

Objectives of Study:

Schizophrenia is a major psychiatric disorder and one of the leading causes of disability. It is characterized by positive and negative symptoms, accompanied by cognitive dysfunction. Identifying objective biomarkers for schizophrenia may inform novel therapeutic strategies and has long been a central focus of psychiatric research. As a precise and non-invasive therapeutic approach, transcranial magnetic stimulation (TMS) has been widely used in psychiatric research and clinical practice. Among its forms, theta-burst stimulation (TBS), a specialized patterned repetitive TMS, has been applied to modulate neural activity, investigate brain function, and serve diagnostic and therapeutic purposes. TMS-EEG is a non-invasive method that enables the assessment and modulation of brain oscillatory activity and brain network connectivity. It can enhance researchers’ and clinicians’ understanding of brain dynamics and brain–behavior relationships in individuals with schizophrenia. To date, no studies have combined TMS-EEG to investigate brain activity in patients with schizophrenia under TBS intervention. Starting from this gap, the present study aims to elucidate the physiological mechanisms underlying changes in the cognitive control network following TBS intervention for schizophrenia at the levels of brain oscillatory activity, functional connectivity, and brain networks.

Inclusion criteria

排除标准:

1.合并其他精神障碍; 2.合并其他全身系统性疾病及脑部器质性病变所致精神病性状态; 3.既往有头颅疾病、脑部损伤、癫痫发作以及其他神经系统疾病史; 4.严重的心血管系统、呼吸系统、免疫系统等全身性疾病史; 5.有酒精、吗啡类药物滥用史; 6.体内有金属植入物如假牙、动脉夹、起搏器或金属固定物等。

Exclusion criteria:

1. Comorbidity with other psychiatric disorders; 2. Psychotic conditions caused by other systemic diseases or organic brain lesions; 3. History of cranial diseases, brain injury, epileptic seizures, or other neurological disorders; 4. History of severe systemic diseases, including those of the cardiovascular, respiratory, and immune systems; 5. History of alcohol or opioid abuse; 6. Presence of metallic implants in the body, such as dentures, arterial clips, pacemakers, or metal fixators.

Blinding:

In this study, a strict blinding protocol was implemented for participants, their families, and all research personnel (including assessors, medical staff, and operators). All clinical assessments were performed by independent raters who were blinded to group allocation. Randomization and the setup of active/sham stimulation were conducted by an independent researcher, and this information was kept confidential from the research implementation team throughout the study.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

采用自制量表收集患者人口学资料和量表资料,采用使用Curry8、SynAmps2和Neuroscan系统采集脑电数据;数据CRF及脑电数据由以为专门的研究人员录入并管理。
