研究目的：

依托新诊断2型糖尿病首年护航健康管理计划的数字化延续管理平台，通过建立前瞻性登记队列，系统评估该数字化管理路径在真实世界环境下的临床结局，为优化糖尿病长期管理策略提供循证依据。  

Objectives of Study：

Relying on the digital continuation management platform of the Escort Health Management Plan in the first year of newly diagnosed type 2 diabetes, by establishing a prospective registration cohort, the clinical outcomes of this digital management path in the real world were systematically evaluated, providing evidence-based basis for optimizing long-term diabetes management strategies.

纳入标准：

Inclusion criteria

排除标准：

1.预期寿命 < 5年或伴有严重的非糖尿病相关合并症（如恶性肿瘤、终末期器官功能衰竭）；
2.严重精神障碍、认知功能障碍或无法配合数字化平台使用者。

Exclusion criteria：

1. Life expectancy < 5 years or accompanied by severe non-diabetes-related comorbidities (such as malignant tumors, end-stage organ failure); 2. Individuals with severe mental disorders, cognitive dysfunction, or those unable to use the digital platform.
