研究目的：

精准评估慢性阻塞性肺疾病（COPD）患者的运动恐惧水平，明确运动恐惧与患者疾病状况、功能限制、社会心理因素的关联，为后续运动恐惧针对性干预方案的开发及临床决策提供科学数据支撑。  

Objectives of Study：

Accurately assess the level of exercise fear among patients with chronic obstructive pulmonary disease (COPD), clarify the associations between exercise fear and patients' disease status, functional limitations, and psychosocial factors, and provide scientific data support for the subsequent development of targeted intervention programs for exercise fear and clinical decision - making.

纳入标准：

Inclusion criteria

排除标准：

1.合并严重心脑血管疾病、肝肾功能衰竭、恶性肿瘤等严重器质性疾病，无法配合完成评估者；
2.存在严重精神障碍、认知功能严重受损，无法正常沟通或理解评估流程者；
3.近期（3个月内）发生 COPD 急性加重，或处于其他疾病急性发作期者；
4.因肢体功能障碍（如严重关节炎、偏瘫等）导致无法进行基本身体活动，影响运动相关评估者；
5.拒绝参与研究或无法配合完成全程评估者。

Exclusion criteria：

1.Individuals with severe organic diseases such as serious cardiovascular and cerebrovascular diseases, liver or kidney failure, or malignant tumors who are unable to cooperate with the assessment process;
2.Individuals with severe mental disorders or significant cognitive impairment who are unable to communicate effectively or comprehend the assessment process;
3.Individuals who have experienced an acute exacerbation of COPD within the past three months, or who are currently experiencing an acute flare-up of another disease;
4.Individuals unable to perform basic physical activities due to physical functional impairments (such as severe arthritis, hemiplegia, etc.), affecting movement-related assessments;
5.Individuals who refuse to participate in the study or are unable to cooperate throughout the entire evaluation process.

在患者完成门诊就诊后或住院期间情绪稳定时，由研究人员发放纸质问卷。患者独立填写，若因视力障碍或书写困难无法自行填写者，由研究人员逐条阅读条目，患者口头回答，研究人员代为填写。所有条目一次性完成，约需5-8分钟。

Patients complete it independently. 数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1. 数据采集 本研究以上海市第二康复医院的COPD稳定期患者人群作为研究对象。通过查阅病历/面对面问卷调查的方式，收集研究对象的一般资料以及暴露因素的相关数据。同时记录研究对象的结局指标。 2. 数据管理 采用Excel建立数据库，所有数据均由两人独立录入并核对，确保准确无误。使用SPSS 26.0统计软件进行数据分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Data CollectionThis study enrolled patients with stable COPD from Shanghai Second Rehabilitation Hospital. General demographic information and exposure-related data were collected through medical record review and face-to-face questionnaires. Outcome measures were concurrently recorded.2. Data ManagementA database was established using Excel. All data were independently entered and cross-checked by two individuals to ensure accuracy. Data analysis was performed using SPSS 26.0 statistical software.
