研究目的：

旨在基于实施科学视角，构建一套适用于老年2型糖尿病患者的药物自我管理干预方案，并在临床实际中验证其可行性及效果，以提升患者的药物自我管理水平与用药依从性，为老年糖尿病规范化用药管理与延续性护理提供循证依据和实践参考。  

Objectives of Study：

The purpose is to construct a set of mdedication self-management intervention programs suitable for elderly patients with type 2 diabetes mellitus based on the perspective of implementation science, and to verify its feasibility and effect in clinical practice, so as to improve the level of medication self-management and medication adherence of patients, and provide evidence-based basis and practical reference for standardized medication management and continuous care of elderly diabetes

纳入标准：

Inclusion criteria

排除标准：

1.研究期间出现严重急性并发症或病情明显恶化者； 2.合并严重心血管、肝肾、神经系统疾病或精神系统疾病，影响自我管理能力者； 3.存在影响药物服用或参与度的其他疾病（如晚期肿瘤等）； 4.正在参加其他药物管理或健康教育干预研究的患者。

Exclusion criteria：

1. Those who have severe acute complications or significantly worsen their condition during the study; 2. Those who have severe cardiovascular, hepatic, renal, neurological or mental system diseases that affect their self-management ability; 3. Other diseases that affect drug use or participation (such as advanced tumors, etc.); 4. Patients who are participating in other drug management or health education intervention studies.
