研究目的：

本课题拟基于网络平台，构建PD患者数字赋能的全程闭环随访管理方案，联合医联体单位进行多级分级协作的资源整合网络及人机协同的精准干预模式，以患者真实需求为中心，护士管理为枢纽，以数据为决策基础，为PD等慢性神经退行性疾病院内-院外管理提供新范式。  

Objectives of Study：

This project aims to build a digital empowerment-based full-cycle closed-loop follow-up management plan for PD patients through a network platform, and to establish a multi-level hierarchical collaboration resource integration network and a human-machine collaborative precision intervention model in conjunction with medical alliance units. Centered on the real needs of patients, with nurse management as the hub and data as the basis for decision-making, it provides a new paradigm for the in-hospital and out-of-hospital management of chronic neurodegenerative diseases such as PD.
