研究目的：

1.识别不同需求特征的社区老年人跌倒预防需求画像，提炼需求 - 资源精准匹配机制，明确多主体协同核心要素、构建协同理论； 2.验证双轮驱动机制的关键要素作用路径； 3.构建 “人工标准处方 + 数字个性处方” 协同社会处方管理模式，开发基于RAG技术的老年跌倒预防 HealthChat 工具并完成可用性测试； 4.通过随机对照试验评估管理模式的健康与成本效益，识别实施过程中的障碍与促进因素; 5.比较护理大语言模型与护士临床决策能力，探究医护人员使用AI辅助工具下的健康管理行为的影响机理及优化策略。  

Objectives of Study：

1. Identify fall prevention demand profiles of community-dwelling older adults with diverse needs, refine the precise demand-resource matching mechanism, clarify the core elements of multi-stakeholder collaboration, and construct a collaboration theory; 2. Verify the action pathways of key components in the two-wheel drive mechanism; 3.Establish a collaborative social prescription management model of "manual standard prescription + digital personalized prescription", develop a fall prevention HealthChat tool for older adults based on RAG technology, and complete usability testing; 4.Evaluate the health and cost-effectiveness of the management model through randomized controlled trials, and identify barriers and facilitators during implementation; 5.Compare the clinical decision-making abilities of nursing large language models and nurses, explore the influencing mechanism and optimization strategies of health management behaviors among healthcare professionals when using AI-assisted tools.

Inclusion criteria

排除标准：

1. 认知严重受损，无法配合干预(画钟试验小于4分)； 2. 三个月内接受心理相关干预，或服用抗抑郁药； 3. 严重运动功能障碍,如瘫痪，需长期卧床； 4. 生命体征不稳定,如晚期肿瘤、严重心肺疾病。

Exclusion criteria：

1. Severe cognitive impairment and inability to cooperate with the intervention (Clock Drawing Test score < 4); 2. Received psychological intervention within three months, or taking antidepressants; 3. Severe motor dysfunction (e.g., paralysis, requiring long-term bed rest); 4. Unstable vital signs (e.g., advanced cancer, severe cardiopulmonary diseases).

认知干预：老年人每周参与社区卫生服务中心老年人认知训练仪器上的2次认知干预。一次15分钟，主要训练内容为完成仪器上的闯关游戏。运动干预依据国际公认的FAME方案开展标准化干预：FAME运动干预通常指 Falls Management Exercise Programme，该方案结合了平衡训练、力量训练、柔韧性训练等多种运动元素，每周干预 2 次，1 次 30 分钟，运动强度依据老年人身体状况调整，持续干预6个月。结合前期开发的大语言模型，开展线下团体干预联合线上个性化健康教育干预。

1.Cognitive Intervention:Older adults participate in 2 cognitive intervention sessions per week using cognitive training devices at community health service centers. Each session lasts 15 minutes, with the main training content consisting of completing level-based games on the devices. 2.Exercise Intervention:The exercise intervention is delivered in a standardized manner in accordance with the internationally recognized FAME protocol: FAME refers to the Falls Management Exercise Programme, which integrates multiple exercise components including balance training, strength training, and flexibility training. The intervention is administered **twice weekly**, with each session lasting 30 minutes. Exercise intensity is adjusted based on the physical condition of older adults, and the intervention lasts for 6 months. Combined with the previously developed large language model, offline group intervention is integrated with online personalized health education intervention.
