研究目的：

评价一种基于血红蛋白动态分层（三档阈值+4周波动）的Triangle分层分级联合“医护共管”新模式，对维持性血液透析患者血红蛋白水平、达标率及变异性的影响。  

Objectives of Study：

To evaluate the effect of a novel Triangle-Stratified Shared Care model—which combines dynamic three-tier hemoglobin-based risk classification (defined by Hb thresholds and 4-week fluctuations) with physician-nurse co-management—on hemoglobin level, standard-reaching rate, and variability in maintenance hemodialysis patients.

纳入标准：

Inclusion criteria

排除标准：

（1）严重营养不良； （2）既往有恶性肿瘤病史； （3）近3个月内存在急性或慢性出血性疾病或接受过输血； （4）近2个月内有严重感染性疾病，或存在控制不佳的慢性炎症性疾病； （5）近3个月内发生不稳定型心绞痛、心肌梗死、接受冠状动脉介入治疗或脑卒中； （6）贫血由非肾脏原因导致（如血液系统恶性肿瘤、凝血功能障碍、溶血性贫血）； （7）研究期间计划接受肾脏移植或重大手术； （8）高血压控制不佳（收缩压＞180 mmHg或舒张压＞100 mmHg）。

Exclusion criteria：

1. severe malnutrition; 2. history of malignancy; 3. acute or chronic hemorrhagic disorders or blood transfusion within the past 3 months; 4. severe infectious diseases within the past 2 months or poorly controlled chronic inflammatory conditions; 5. unstable angina, myocardial infarction, coronary intervention, or stroke within the past 3 months; 6. anemia secondary to nonrenal causes (e.g., hematologic malignancies, coagulopathies, hemolytic anemia); 7. planned kidney transplantation or major surgery during the study period; 8. poorly controlled hypertension (systolic blood pressure >180 mm Hg or diastolic blood pressure >100 mm Hg).

During the intervention phase (February 2024 to February 2025), the "Triangle-Stratified Grading + Shared Doctor-Nurse Care" model was implemented: dynamic stratification based on hemoglobin (Hb) levels (low, medium, high risk, combined with Hb fluctuations within 4 weeks), followed by individualized anemia management plans developed jointly by physicians and nurses, including adjustments of erythropoietin dosage, iron supplementation regimens, nutritional guidance, and follow-up plans.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集使用自行设计的病例记录表（CRF），从医院电子病历系统中提取患者的人口学特征、临床检验指标（血红蛋白、铁蛋白、白蛋白等）、用药记录（促红细胞生成素剂量、铁剂用量）及相关临床事件。所有数据由两名研究人员独立录入Excel数据库，并进行一致性核对。未使用专业的电子数据采集系统（EDC）。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data were collected using a self-designed Case Record Form (CRF). Patient demographics, laboratory parameters (hemoglobin, ferritin, albumin, etc.), medication records (erythropoietin dosage, iron supplementation), and clinical events were extracted from the hospital's electronic medical record system. All data were independently entered into an Excel database by two researchers and cross-checked for consistency. No professional Electronic Data Capture (EDC) system was used.
