研究目的：

全面了解衡阳市基层医疗机构在慢阻肺预防、筛查、诊断、治疗、管理及健康教育等方面的现状，明确其优势与面临的挑战；收集基层医务工作者对慢阻肺公卫项目政策支持的需求，为卫生行政部门优化基层慢阻肺防治策略、合理配置医疗资源、完善培训体系提供科学依据，进而提升区域内基层慢阻肺防治水平，惠及广大慢阻肺患者。  

Objectives of Study：

Comprehensively understand the current situation of grassroots medical institutions in Hengyang City in terms of prevention, screening, diagnosis, treatment, management and health education of chronic obstructive pulmonary disease (COPD), and clarify their advantages and challenges; collect the demands of grassroots medical workers for policy support for the public health project on COPD, to provide a scientific basis for the health administrative department to optimize the prevention and treatment strategies for COPD at the grassroots level, rationally allocate medical resources, and improve the training system, thereby enhancing the prevention and treatment level of COPD at the grassroots level in the region and benefiting the vast number of COPD patients.

Inclusion criteria

排除标准：

不在衡阳市基层医疗机构工作的人员。工作内容与慢阻肺诊疗、管理无关的人员。因认知障碍、语言障碍等无法独立完成问卷填写的人员。 明确拒绝参与本次研究的人员。

Exclusion criteria：

Those who do not work in the grassroots medical institutions of Hengyang City. Those whose job duties have nothing to do with the diagnosis and management of chronic obstructive pulmonary disease. Those who are unable to independently complete the questionnaire due to cognitive or language impairments. Clear. Those who explicitly refused to participate in this study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.数据采集： 使用“问卷星”或类似平台制作电子问卷，或发放统一格式的 纸质问卷。 2. 数据库建立与录入： 使用 EpiData 或 Excel 建立数据库。电子数据直接导 出，纸质问卷由经过培训的研究人员采用双人独立录入方式进行录入，并 进行一致性校验。 3. 数据存档： 所有电子数据存储在机构配备的、有密码保护的计算机中。纸 质问卷保存于南华大学附属第二医院呼吸科教研室的上锁文件柜中。研究 结束后，数据将继续保密保存 5 年以备核查。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection: Use "Questionnaire Star" or similar platforms to create electronic questionnaires, or distribute uniform-format paper questionnaires. 2. Database establishment and entry: Use EpiData or Excel to establish the database. Electronic data are directly exported, and the paper questionnaires are entered by trained researchers using a double-person independent entry method and consistency verification is conducted. 3. Data archiving: All electronic data are stored in password-protected computers equipped by the institution. Paper questionnaires are kept in the locked filing cabinet of the Respiratory Department Teaching and Research Room of the Second Affiliated Hospital of South China University of Technology. After the study is completed, the data will be kept confidentially for another 5 years for verification purposes.
