研究目的：

重症肺炎患者常因感染负荷重、代谢需求高及器官功能受损而处于显著的营养消耗和生理应激状态，常伴随喂养耐受性差、肠道屏障受损和肌力下降，这些因素均会影响机械通气脱机、炎症控制及整体预后。尽管肠内营养被指南推荐为重症患者营养支持的首选途径，但关于氨基酸型肠内营养制剂在改善营养达标率、提升喂养耐受性、改善肠道功能及促进临床恢复方面的优势，目前仍缺乏明确且高质量的临床证据。为弥补这一研究空白，本研究旨在通过与短肽制剂的对照，系统评估氨基酸型肠内营养在重症肺炎患者中的应用效果，以期为临床制定更科学、精准的营养治疗策略提供可靠依据。  

Objectives of Study：

Patients with severe pneumonia often experience substantial metabolic stress, impaired organ function, feeding intolerance, disruption of the intestinal barrier, and progressive loss of muscle strength, all of which adversely influence weaning from mechanical ventilation, inflammation control, and overall clinical outcomes. Although enteral nutrition is recommended as the preferred strategy in critically ill patients, robust evidence demonstrating whether amino acid–based enteral formulas offer superior benefits—such as improved nutritional adequacy, better feeding tolerance, enhanced intestinal integrity, and accelerated clinical recovery—remains limited. To address this gap, this study aims to systematically evaluate the effectiveness of amino acid–based enteral nutrition compared with short-peptide formulas in patients with severe pneumonia, thereby providing scientific evidence to guide more precise and optimized nutritional therapy in critical care practice.

纳入标准：

Inclusion criteria

排除标准：

1.对肠内营养中成分有过敏史者；
2.心、肝、肾严重障碍、内环境严重紊乱者；
3.重症糖尿病、大量服用类固醇的患者；
4.妊娠期及哺乳期妇女；
5.氨基酸代谢异常的患者；
6.休克未控制，血流动力学尚未稳定或组织灌注不足；
7.未控制的致命性低氧血症、高碳酸血症或酸中毒；
8.存在上消化道活动性出血、显著的胃肠道缺血、高流量小肠瘘、腹腔间隔室综合征；
9.胃肠减压引流量>500 ml/6 h；
10.其他医护人员认为不适于参与本研究者。

Exclusion criteria：

1.Patients with a history of allergy to any component of enteral nutrition formulas;
2.Patients with severe cardiac, hepatic, or renal dysfunction, or with significant disturbances in internal homeostasis;
3.Patients with severe diabetes or those receiving high-dose corticosteroid therapy;
4.Pregnant or lactating women;
5.Patients with disorders of amino acid metabolism;
6.Patients with uncontrolled shock, hemodynamic instability, or inadequate tissue perfusion;
7.Patients with uncontrolled life-threatening hypoxemia, hypercapnia, or acidosis;
8.Patients with active upper gastrointestinal bleeding, significant gastrointestinal ischemia, high-output small intestinal fistula, or abdominal compartment syndrome;
9.Patients with gastrointestinal decompression drainage exceeding 500 mL per 6 hours;
10.Patients deemed unsuitable for participation by the clinical care team.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究的数据采集采用统一设计的病例记录表（CRF），由经过培训的研究人员根据患者的临床信息、检查结果及随访数据进行填写。所有数据将同步录入电子数据采集系统（Electronic Data Capture, EDC），系统设置访问权限控制、自动逻辑检查及数据追溯功能，以确保数据的完整性和准确性。研究数据将由专职数据管理员定期核查并进行质控，最终在数据锁定后进行统计分析。研究过程中所有数据均按照医院数据安全管理要求进行存储和备份，不包含任何可识别患者身份的信息。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data will be collected using a standardized Case Report Form (CRF) completed by trained study personnel based on clinical information, laboratory results, and follow-up assessments. All data will be entered into an electronic data capture (EDC) system equipped with access control, automated logical checks, and audit trails to ensure data integrity and accuracy. A designated data manager will perform regular data monitoring and quality control. After data verification and database lock, the dataset will be used for statistical analysis. All study data will be stored and backed up in compliance with hospital data security regulations, with no personally identifiable patient information included.
