比较不同模式下、传统DLPFC靶点与fMRI功能连接引导的个性化靶点对阿尔茨海默病(AD)患者认知功能的调控效应,并基于量表-血液-脑电-功能核磁多模态数据探究不同方案TMS的调控机制  

Objectives of Study:

Compare the regulatory effects on cognitive function in Alzheimer's disease (AD) patients between traditional DLPFC targets and fMRI-guided personalized targets across different protocols. Investigate the regulatory mechanisms of TMS across different protocols based on multi-modal data integrating questionnaires, blood tests, EEG, and functional MRI.

纳入标准:

Inclusion criteria

排除标准:

1.存在癫痫病史或者其他神经系统疾病史; 2.具有24项汉密尔顿抑郁评定量表(Hamilton Depression Scale,HAMD)上得分>20的严重抑郁症; 3.有视听觉障碍; 4.有任何严重的慢性全身性疾病史、脑损伤史、酒精或药物滥用史; 5.有任何TMS的禁忌症。

Exclusion criteria:

1. Has a history of epilepsy or other neurological disorders; 2. Has severe depression with a score of >20 on the 24-item Hamilton Depression Rating Scale (Hamilton Depression Scale, HAMD); 3. Has visual or auditory impairment; 4. Has any serious chronic systemic disease history, brain injury history, history of alcohol or drug abuse; 5. Has any contraindications for TMS.

This study employs a centralized randomization system for subject allocation: each enrolled subject will receive a unique study identification code; an independent statistician generates the allocation sequence using block randomization, with strict allocation concealment implemented (sealed envelope method).

盲法:

实施单盲设计以最小化实施偏倚与测量偏倚:受试者通过使用高仿真伪刺激线圈保持盲态,所有疗效评估者及数据分析师对分组信息均不知情。尽管刺激操作员因参数差异无法设盲,但其不参与评估且沟通受限。

Blinding:

A single-blind design was implemented to minimize implementation bias and measurement bias: Subjects maintained blindness by using high-fidelity sham stimulation coils, while all efficacy evaluators and data analysts remained unaware of group assignments. Although stimulation operators could not be blinded due to parameter differences, they did not participate in evaluations and had restricted communication.
