研究目的：

本研究旨在通过随机对照试验，评估重复经颅磁刺激（rTMS）对脑卒中后膈肌功能障碍患者膈肌功能及呼吸功能的临床疗效，并观察其对血清脑源性神经营养因子（BDNF）水平的影响，进一步探索BDNF基因多态性对rTMS治疗效果的潜在调节作用。  

Objectives of Study：

The objective of this study is to evaluate the clinical efficacy of repetitive transcranial magnetic stimulation (rTMS) on diaphragmatic and respiratory function in patients with post-stroke diaphragmatic dysfunction through a randomized controlled trial, to investigate its effects on serum brain-derived neurotrophic factor (BDNF) levels, and to explore the potential modulatory role of BDNF gene polymorphisms on the therapeutic response to rTMS.

纳入标准：

Inclusion criteria

排除标准：

1.患有慢性阻塞性肺疾病、肺结核和肺癌等呼吸系统疾病者； 2.病情恶化，生命体征不稳定者； 3.小脑及脑干卒中患者； 4.合并重症肌无力或膈神经麻痹者； 5.患者存在重要器官功能严重受损或病情危重者； 6.体内心脏起搏器植入、癫痫、患侧颅骨缺损者等有TMS治疗禁忌症者。

Exclusion criteria：

1. Patients with respiratory system diseases such as chronic obstructive pulmonary disease, pulmonary tuberculosis, and lung cancer; 2. Patients whose condition has worsened and vital signs are unstable; 3. Patients with cerebellar and brainstem stroke; 4. Patients with comorbid myasthenia gravis or phrenic nerve paralysis; 5. Patients with severe impairment of important organ functions or critical illness; 6. Patients with contraindications to TMS treatment, such as implanted cardiac pacemakers, epilepsy, or cranial defects on the affected side.

Randomization Procedure (please state who generates the random number sequence and by what method)：

Participants will be randomly assigned in a 1:1 ratio using a random number table. Allocation concealment will be ensured by the use of sequentially numbered, opaque, sealed envelopes. Both participants and outcome assessors will be blinded to group assignment. The control group will receive sham stimulation designed to mimic active treatment, in order to minimize selection bias, placebo effects, and assessment bias, and to enhance the objectivity and reliability of the study outcomes.
