研究目的：

本研究拟通过高效液相色谱-串联质谱技术比较HDP产妇、正常产妇在布比卡因联合芬太尼腰麻后的胎盘转运情况以及精确测定HDP产妇母体血药浓度。  

Objectives of Study：

This study aims to employ high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) to compare the placental transfer of drugs following spinal anesthesia with bupivacaine and fentanyl between parturients with hypertensive disorders of pregnancy (HDP) and normotensive parturients, and to accurately determine the maternal plasma concentrations in the HDP group.

纳入标准：

Inclusion criteria

排除标准：

（1）椎管内麻醉禁忌证； （2）对局麻药或阿片类药物过敏 （3）胎儿宫内窘迫需紧急手术 （4）严重肝肾功能异常（ALT>正常值2倍，Cr>120μmol/L） （5）合并心脏疾病、认知功能障碍无法配合研究 （6）患者拒绝参加研究

Exclusion criteria：

(1) Contraindications to neuraxial anesthesia. (2) Known allergy to local anesthetics or opioids. (3) Fetal distress requiring emergent surgery. (4) Significant hepatic or renal dysfunction (ALT > 2 times the upper limit of normal, Cr > 120 μmol/L). (5) Comorbid cardiac disease or cognitive impairment precluding study cooperation. (6) Patient refusal to participate.
