研究目的：

描述脑卒中患者出院后 6 个月、1 年手功能及整体康复的动态轨迹；分析多学科协作对运动皮质神经可塑性的调控作用；验证多学科管理模式对并发症、康复满意度的影响；构建适配我国基层医疗的连续康复管理模式。  

Objectives of Study：

To describe the dynamic trajectory of hand function and overall rehabilitation in stroke patients at 6 months and 1 year after discharge; to analyze the regulatory effect of multidisciplinary collaboration on motor cortical neuroplasticity; to verify the impact of multidisciplinary management mode on complication rate and rehabilitation satisfaction; to construct a continuous rehabilitation management mode adapted to China's primary medical care.

纳入标准：

Inclusion criteria

排除标准：

1.　存在严重认知障碍、失语或精神疾病，无法有效沟通； 2.　既往有神经系统疾病或严重骨关节病，影响手功能评估； 3.　合并重要脏器严重功能障碍、恶性肿瘤或严重感染； 4.　妊娠或哺乳期女性； 5.　出院后计划迁出合肥市，无法完成随访； 6.　近 1 个月内参与其他同类康复研究； 7.　存在 MEP 检测禁忌证（如体内金属植入物、癫痫病史等）。

Exclusion criteria：

1. Severe cognitive impairment, aphasia, or mental illness, making effective communication impossible; 2. A history of neurological diseases or severe osteoarthropathy that affects the assessment of hand function; 3. Complicated with severe dysfunction of major organs, malignant tumors, or severe infections; 4. Pregnant or lactating women; 5. Planning to move out of Hefei after discharge and unable to complete follow-up; 6. Participation in other similar rehabilitation studies within the past month; 7. Having contraindications to MEP testing (such as internal metal implants, history of epilepsy, etc.).
