研究目的:

2.1.1 主要目标 (1)基于可视化平台针对标准STN"运动甜点"程控及传统试错法程控开发基于症状网络图谱的个体化程控算法; (2)建立一套融合影像与网络图谱的标准化、可视化DBS程控体系; (3)在前瞻性队列中,比较基于症状网络图谱的"网络融合"程控、基于标准STN"运动甜点"的可视化程控以及传统经验程控,对PD患者运动症状改善(UPDRS-III评分)的有效性; 2.1.2 次要目标 (1)评估三种程控策略在改善患者生活质量(PDQ-39)、非运动症状、轴性症状等方面的差异; (2)比较三种程控策略的程控效率(程控耗时、随访次数)和患者满意度; (3)系统评估采用8触点MICC电极对多核团(ZI头端、STN、SNr)进行刺激的安全性及副作用谱; (4)探索不同PD临床亚型(震颤主导型/TD、强直-少动型/BSID、姿势不稳步态障碍主导型/PIGD)对不同程控策略的治疗反应。  

Objectives of Study:

2.1.1 Main Objectives (1) Develop an individualized programming algorithm based on the symptom network map for the standard STN "motion sweet spot" programming and the traditional trial-and-error method, using a visualization platform; (2) Establish a standardized and visualized DBS programming system that integrates imaging and network maps; (3) In a prospective cohort study, compare the effectiveness of "network fusion" programming based on symptom network maps, visual programming based on the standard STN "motion sweet spot", and traditional empirical programming in improving motor symptoms (UPDRS-III score) in PD patients; 2.1.2 Secondary Objectives (1) Evaluate the differences in the three programming strategies in improving patients' quality of life (PDQ-39), non-motor symptoms, and axial symptoms; (2) Compare the programming efficiency (programming time, follow-up frequency) and patient satisfaction of the three programming strategies; (3) Systematically evaluate the safety and side effect spectrum of stimulating multiple nuclei (ZI head end, STN, SNr) with 8-contact MICC electrodes; (4) Explore the treatment responses of different PD clinical subtypes (dystonia-dominant type/TD, rigidity-hypokinesia type/BSID, postural instability and gait disorder-dominant type/PIGD) to different programming strategies.

(1)继发性帕金森综合征 (2)严重认知障碍(MoCA<20) (3)严重精神疾病、手术禁忌证 (4)不能配合完成随访评估

(1) Secondary Parkinsonism (2) Severe cognitive impairment (MoCA < 20) (3) Severe mental illness, surgical contraindications (4) Unable to cooperate in completing follow-up assessment
