研究目的：

（1）制定城镇街道综合减盐干预措施。 （2）评估干预策略12个月的有效性和24个月的减盐效果。 （3）评估12个月和24个月干预的成本效益。  

Objectives of Study：

(1) To Develop comprehensive measures for reducing salt in urban streets. (2) To evaluate the effectiveness of intervention strategy for 12 months and the effect of salt reduction for 24 months. (3) To assess the cost-effectiveness of intervention in 12 months and 24 months.

药物成份或治疗方案详述：

干预组: 支持环境发展：如海报，报纸，公共广告和本地广播，传递减盐核心信息。重点人群培训：开发PowerPoint或视频等培训资料，培养政府和卫生领导，卫生机构，职工等重点人群。以社区为基础的健康传播活动：传播健康宣传资料，研讨会和活动，为家庭主妇提供限盐勺，食物秤等辅助工具，帮助形成健康的饮食习惯。 推广使用AppSalt（基于手机的APP）进行在线健康教育，定期评估家庭的食盐摄入量。基于学校的健康交流活动：创建包括教室、校园和食堂在内的减盐环境，提供AppSalt利用指导，促进和监督使用。以餐饮为主的健康传播活动：由疾控中心进行一至两次餐饮业领导和厨师培训，推广餐厅低盐菜。 在餐厅提供减盐核心信息。 推广和使用低盐菜单。乡村诊所减盐活动：依托国家基本公共卫生服务，医生对高血压和高危人群进行减盐干预。  

Description for medicine or protocol of treatment in detail：

Intervention group: 1. Supportive environment developmentSuch as posters, newspaper, public advertisement and local broadcast to deliver salt reduction core messages; 2. Training for key population: develop training material such as PowerPoint or video, training key people group, such as government and health leaderships, Health Institutes and Staff; 3. Community-based Health Communication Activities: Disseminate Health communication materials, Workshop and activities, providing housewives with supporting tools such as salt restriction spoon, food scale to help to form healthy diet habit. Promote and using AppSalt (Mobile-based APP) to conduct online health education and assessment; 4. School-based Health Communication Activities: create salt-reduction environment including classroom, campus and canteens, and provide guidance on AppSalt utilisation, promote and surveillance using; 5. Restaurants-based Health Communication Activities: Conduct once or twice restaurant leaders and chefs training by CDC to promote restaurant support low-salt dish. Deliver salt reduction core messages in restaurants. Promote and using Menu for reducing salt and low-salt dishes; 6. Village clinic-based Salt Reduction Activities: Relying on national basic public health services, Doctors conduct salt reduction intervention for hypertension and high-risk people. Control group: No additional intervention on salt reduction will be given to control group in year 1. In year 2, intervention strategies will be made available to general public, including those originally randomized to the control group. All the participants in this study will be encouraged to follow salt reduction strategy. 

纳入标准：

Inclusion criteria

排除标准：

（1）孕妇或哺乳期妇女
（2）目前正在参加任何其他的临床试验
（3）任何可能影响干预和后续行动的严重精神或身体疾病
（4）符合下列条件之一的24小时尿液采样将被剔除：
a）因小便失禁导致不能收集尿液;
b）候选人难以收集尿液，找不到助手;
c）急性/慢性尿路感染，阴道感染和肛周感染的患者;
d）泌尿道、阴道和消化道急性出血性疾病患者;
e）严重呕吐和腹泻症状的患者。

Exclusion criteria：

1. Women in pregnant or lactation period;
2. Currently participate in any other clinical trial;
3. Any severe psychiatric or physical diseases which might impact intervention and follow up;
4. Samples for 24-hour urine collection will be rejected if anyone satisfying any of the following conditions:
1) Urine cant be collected due to aconuresis;
2) The candidate has difficulty to collect urine and cant find an assistant;
3) Patients with acute/chronic urinary tract infection, vaginal infection and perianal infection;
4) Patients with acute hemorrhagic diseases in urinary tract, vagina and digestive tract;
5) Patients with severe vomiting and diarrheic symptoms.
