评估与比较艾玛昔替尼与夫那奇珠单抗在活动性大动脉炎治疗中的疗效及安全性。  

Objectives of Study：

Comparing the efficacy and safety for vunakizumab and ivarmacitinib in the treatment of active Takayasu’s arteritis (TAK)

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1.病情严重或重要脏器受累，需限期或立即进行手术治疗者；
2.重要脏器受累，例如心肌或冠脉受累者，预计研究期间需要对应加强治疗；
3.活动性感染，包括结核、乙型肝炎病毒、丙型肝炎病毒、HIV 病毒、细菌 或真菌感染（潜伏结核感染患者可在入组的同时接受预防性抗结核治疗）；
4.入组前 5 年内有恶性肿瘤病史或入组时有仍未治愈的恶性肿瘤；
5.控制不佳的糖尿病；
6.有静脉血栓病史；
7.入组前三个月内有上消化道出血的患者；
8.入组时伴有控制不佳的心脏功能衰竭；
9.研究者认为具有任何可能导致受试者不能完成本研究或给受试者带来明显 风险的其他医疗、精神和/或社会因素；
10.既往曾使用 JAK 抑制剂或白介素 17a抑制剂或该类药物的类似药品；
11.已妊娠或研究期间有妊娠意向者；
12.严重肾功能不全（eGFR＜30ml/min）；
13.严重的肝脏病变，定义为血清 ALT或 AST高于正常上限 3 倍以上；
14.难治性高血压；

Exclusion criteria：

1.Severe disease or involvement of critical organs requiring urgent or immediate surgical intervention;
2.Involvement of major organs, such as myocardial or coronary artery involvement, for which intensified or corresponding treatment is anticipated during the study period;
3.Active infections, including tuberculosis, hepatitis B virus, hepatitis C virus, HIV infection, bacterial or fungal infections (patients with latent tuberculosis infection may receive prophylactic anti-tuberculosis therapy at the time of enrollment);
4.History of malignancy within 5 years prior to enrollment, or presence of an active, unresolved malignancy at the time of enrollment;
5.Poorly controlled diabetes mellitus;
6.History of venous thromboembolism;
7.Upper gastrointestinal bleeding within 3 months prior to enrollment;
8.Poorly controlled heart failure at the time of enrollment;
9.Refractory hypertension;
10.Severe hepatic impairment, defined as serum alanine aminotransferase (ALT) or aspartate aminotransferase (AST) > 3 times the upper limit of normal (ULN);
11.Severe renal insufficiency (eGFR < 30 mL/min);
12.Pregnant women or those planning pregnancy during the study period;
13.Prior use of Janus kinase (JAK) inhibitors or interleukin-17A inhibitors, or biosimilars/agents of the same class;
14.Any other medical, psychiatric, and/or social conditions that, in the investigator’s opinion, may prevent the subject from completing the study or pose a significant risk to the subject;
