研究目的：

本研究旨在精确测定该联合方案中瑞芬太尼的 ED₅₀，为该类特殊人群提供一种安全、精准的麻醉镇痛策略，并为临床指南提供高级别循证医学证据。  

Objectives of Study：

This study aims to accurately determine the ED₅₀ of remifentanil in this combined regimen, providing a safe and precise anesthetic analgesia strategy for this special population, and offering high-level evidence-based medical data for clinical guidelines.

纳入标准：

Inclusion criteria

排除标准：

1.对阿片类药物或酰胺类局部麻醉药过敏；严重心、肺、肝、肾功能不全；长期使用阿片类或苯二氮䓬类镇痛镇静药物；困难气道评估（Mallampati分级IV级、张口度<3cm等）；严重心、肝、脑、肺、肾、凝血功能障碍者；存在精神或认知障碍，无法配合评估。

Exclusion criteria：

1.Allergy to opioids or amide-type local anesthetics;
2.Severe impairment of cardiac, pulmonary, hepatic, or renal function;
3.Chronic use of opioid or benzodiazepine analgesics/sedatives;
4.Anticipated difficult airway (e.g., Mallampati class IV, inter-incisor distance <3 cm);
5.Severe dysfunction of the heart, liver, brain, lungs, kidneys, or coagulation system;
6.Presence of psychiatric or cognitive disorders that preclude cooperation with assessments.
