研究目的:

本研究为诊断性研究,主要内容为基于人工智能和18F-FDG PET影像构建额叶癫痫(FLE)患者代谢癫痫网络机制,阐明癫痫与癫痫网络之间的机制,开发并验证一种人工智能算法模型,实现对癫痫起源灶及患者术后结局的预测,为临床医生提供精准术前指导。  

Objectives of Study:

This research is a diagnostic study. Its main content is to construct the metabolic epilepsy network mechanism of patients with frontal lobe epilepsy (FLE) based on artificial intelligence and 18F-FDG PET images, clarify the mechanism between epilepsy and the epilepsy network, develop and validate an artificial intelligence algorithm model, and achieve the prediction of the origin site of epilepsy and the postoperative outcome of patients, providing precise preoperative guidance for clinicians.

排除标准: 1.伴有脑炎、脑积水、脑出血、脑血管疾病、颅内占位性病变; 2.图像质量差,如头部运动引起严重图像伪影情况、扫描不规范导致图像显像不理想; 3.诊断为认知障碍的患者。

Exclusion criteria: 1. Patients accompanied by encephalitis, hydrocephalus, cerebral hemorrhage, cerebrovascular diseases, or intracranial space - occupying lesions; 2. Patients with poor image quality, such as those with severe image artifacts caused by head movement or those with sub - optimal image visualization due to non - standard scanning; 3. Patients diagnosed with cognitive impairment.

金标准或参考标准(即可准确诊断某疾病的单项方法或多项联合方法,在本研究中用于诊断是否有该病的临床参考标准):

目前对于额叶癫痫致痫灶的定位是通过综合分析临床发作模式,脑电图放电模式,MRI检查,18F-FDG PET检查等结果,来指明具体致痫灶,最重要和相对比较权威的参考标准是:具有侵入性的检查SEEG检查(即颅内电极植入),通过植入的颅内电极捕捉患者癫痫发作时的放电模式,从而判断具体的致痫灶。同时也根据患者术后癫痫症状是否复发来评估术前定位并已切除的致痫灶是否是引起患者癫痫发作的实际病灶。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Currently, the localization of epileptogenic foci in frontal lobe epilepsy is achieved through comprehensive analysis of the results of clinical seizure patterns, electroencephalogram (EEG) discharge patterns, magnetic resonance imaging (MRI) examinations, 18F - FDG positron emission tomography (PET) examinations, etc., to identify specific epileptogenic foci. The most important and relatively authoritative reference standard is the invasive stereoelectroencephalography (SEEG) examination (i.e., intracranial electrode implantation). By using the implanted intracranial electrodes to capture the discharge patterns during the patient's epileptic seizures, the specific epileptogenic foci can be determined. Meanwhile, based on whether the patient's epileptic symptoms recur after surgery, an assessment is made to determine whether the pre - operatively localized and resected epileptogenic foci are the actual lesions causing the patient's epileptic seizures.

Index test:

This study analyzes the brain MRI and 18F - FDG PET imaging data of frontal lobe patients and healthy individuals. By integrating artificial intelligence to analyze the metabolic brain network, it provides suggestions regarding possible epileptogenic foci. Subsequently, these suggestions are compared with the actual epileptogenic foci identified through SEEG examination and then resected. Finally, the accuracy of the model in analyzing and determining epileptogenic foci is obtained.

额叶癫痫患者多在夜间睡眠中突发<1分钟的尖叫-吼叫伴非对称强直或过度运动(踢腿、蹬车、骨盆扭动),发作频繁而刻板,意识常部分保留、无发作后迷糊,视频-EEG示额区起始高频快节律,60%药物难治,但MRI/PET阳性且可完全切除者术后近七成可获长期无发作。

Target condition (The target condition is a particular disease or disease stage that the index test will be intended to identify. Please specify the characteristics in detail; the population should has a complete spectrum and good representative):

Patients with frontal lobe epilepsy often experience sudden screams and roars lasting less than 1 minute accompanied by asymmetric tonic contractions or excessive movements (such as kicking, pedaling, and pelvic writhing) during nocturnal sleep. The attacks are frequent and stereotyped. Consciousness is often partially preserved, and there is no post - ictal confusion. Video - EEG shows a high - frequency fast rhythm originating from the frontal region. Approximately 60% of the patients are refractory to drug treatment. However, among those with positive MRI/PET findings whose lesions can be completely resected, nearly 70% can achieve long - term seizure - freedom after surgery.
