研究目的：

比较减重手术患者在麻醉恢复室拔管后，采用不同半卧位角度（15° vs. 30° vs. 45°）10分钟内低氧血症（SpO2≤90%）的发生率，探索最佳半卧角度对氧合功能的保护作用。  

Objectives of Study：

To compare the incidence of hypoxemia (SpO2 <= 90%) within 10 minutes after extubation in the anesthesia recovery room among patients undergoing weight loss surgery who were placed in different semi-recumbent positions (15°, 30°, and 45°), and to explore the protective effect of the optimal semi-recumbent position angle on oxygenation function.

纳入标准：

Inclusion criteria

排除标准：

（1）术前存在低氧血症或呼吸功能障碍（SpO2 < 90%（静息状态，吸空气）术前需长期氧疗或无创通气）； （2）血流动力学不稳定（HR<50 次/分或HR>120 次,SBP<90mmHg或需要血管活性药物维持）； （3）严重脊柱畸形、骨折、神经系统疾病导致体位受限； （4）孕妇和哺乳期妇女； （5）佩戴心脏起搏器、除颤器和其他有源电动植入物的患者； （6）近三个月内参加其他临床研究；

Exclusion criteria：

(1) Preoperative hypoxemia or respiratory dysfunction (SpO2 < 90% (at rest, breathing air), requiring long-term oxygen therapy or non-invasive ventilation before surgery); (2) Hemodynamic instability (HR < 50 beats per minute or HR > 120 beats per minute, SBP < 90 mmHg, or requiring maintenance with vasoactive drugs); (3) Severe spinal deformities, fractures, or neurological diseases leading to limited body position; (4) Pregnant and lactating women; (5) Patients with cardiac pacemakers, defibrillators, and other active electrical implants; (6) Participation in other clinical studies within the past three months;

由独立统计师使用 Excel 2021 RAND() 函数生成 447 个 0–447之间随机小数，按从小到大排序后，1-149号为“15°半卧位组”，149-298号为“30°半卧位组”，298-447号为“45°半卧位组”，随机序列保存于加密 U 盘。随机序列打印后装入连续编号、不透光、无标识信封内；信封由统计师封存于专柜，受试者入室后由麻醉护士按顺序拆封，拆封过程双人照相留痕 。

Randomization Procedure (please state who generates the random number sequence and by what method)：

An independent statistician generated 447 random decimal numbers between 0 and 447 using the Excel 2021 RAND() function. After sorting them in ascending order, numbers 1-149 were assigned to the "15° semi-reclining position group", numbers 149-298 to the "30° semi-reclining position group", and numbers 298-447 to the "45° semi-reclining position group". The random sequence was saved on an encrypted USB drive. The random sequence was printed and placed in consecutively numbered, opaque, unmarked envelopes; the envelopes were sealed by the statistician in a designated cabinet. After the subjects entered the room, the anesthesiologist unsealed the envelopes in order, and the unsealing process was photographed by two people for record-keeping.
