研究目的:

1. 构建慢性咳嗽患者的“呼出气挥发性有机化合物气味指纹库”,建立并验证慢性咳嗽分类诊断模型; 2. 进一步优化电子鼻设备,实现慢性咳嗽病因诊断新技术临床转化和基层推广。  

Objectives of Study:

1. Build an "exhaled volatile organic compound odor fingerprint database" for patients with chronic cough, and establish and validate a classification and diagnostic model for chronic cough; 2. Further optimize electronic nose devices to achieve clinical translation and grassroots promotion of new diagnostic technologies for the causes of chronic cough.

纳入标准:

Inclusion criteria

排除标准:

1.正在吸烟或已戒烟但<2年; 2.处于妊娠期或哺乳期; 3.8周内有急性上呼吸道感染史; 4.4周内口服或吸入了糖皮质激素; 5.患有严重心功能不全、严重肝肾功能障碍、精神及认知功能障碍、听力及交流障碍等。

Exclusion criteria:

1.Currently smoking or quit smoking for less than 2 years; 2. Pregnant or lactating; 3. History of acute upper respiratory infection within 8 weeks; 4. Oral or inhaled glucocorticoids within 4 weeks; 5. Severe heart failure, severe liver/kidney dysfunction, mental/cognitive impairment, or hearing/communication disorders.

由于慢性咳嗽病因多样且常常重叠,目前尚不存在单一的“金标准”诊断方法,而是通过‍“排除法”‍结合特定病因的特异性检查来最终明确诊断。以下是基ERS指南和中华医学会呼吸病学分会指南的详细诊断策略: 1.初步评估(排除法):病史+基础影像学检查(胸部X光)+ 实验室检查(血常规、鼻窦X光或CT); 2.病因特异性检查:怀疑CVA 进行支气管激发试验、怀疑GERC进行24h食管pH阻抗监测、怀疑EB进行诱导痰嗜酸性粒细胞计数等 3.治疗反应评估:给予经验性治疗,根据治疗反应推断病因诊断 慢性咳嗽的诊断遵循排除-确认-治疗反应的路径

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The diagnostic approach for chronic cough is primarily rooted in an exclusionary framework, supplemented by disease-specific investigations to achieve a definitive diagnosis. According to the ERS and Chinese Thoracic Society guidelines, the evaluation process is systematically categorized into three integral stages: 1. Initial Assessment (Exclusionary Process): Comprehensive History: A detailed clinical history to identify potential triggers and associated factors. Baseline Imaging: Chest radiography serves as a fundamental screening tool to rule out serious underlying pathologies. Laboratory Investigations: Routine blood tests and sinus imaging (X-ray or CT) are employed to exclude systemic and ENT-related contributors. 2. Disease-Specific Diagnostics: Cough Variant Asthma (CVA): Bronchial provocation testing is employed to assess airway hyperresponsiveness. Gastroesophageal Reflux Cough (GERC): 24-hour esophageal pH-impedance monitoring is utilized to detect both acid and non-acid reflux events. Eosinophilic Bronchitis (EB): Induced sputum eosinophil counts are measured to identify airway eosinophilic inflammation. 3. Therapeutic Response Evaluation: Empirical Treatment: A structured trial of targeted therapy is administered. Response Assessment: The therapeutic outcomes are closely monitored to infer the underlying etiology and refine the diagnosis. In essence, the diagnostic paradigm for chronic cough follows a structured "exclusion-confirmation-therapeutic response" pathway, integrating both systematic screening and targeted investigations to ensure diagnostic accuracy.
