研究目的:

本研究旨在系统阐明CXCL12-CXCR4趋化轴在斑秃发病机制中的关键作用,聚焦成纤维细胞和免疫细胞互作以及CXCL12是否通过介导CXCR4⁺CD8⁺T细胞的趋化与激活破坏毛囊免疫耐受,推动疾病进展。通过单细胞转录组测序在不同病程阶段对CXCL12-CXCR4表达水平与功能状态进行动态描绘,并结合功能阻断实验和人类标本验证,该信号轴是否能作为斑秃的治疗靶点及病程状态和预后的生物标志物,探索其作为时程特异治疗靶点的可能性。  

Objectives of Study:

This study aims to systematically elucidate the pivotal role of the CXCL12–CXCR4 chemotactic axis in the pathogenesis of alopecia areata, with a particular focus on fibroblast–immune cell interactions and on whether CXCL12 disrupts hair follicle immune privilege and drives disease progression by mediating the chemotaxis and activation of CXCR4⁺CD8⁺ T cells. Using single-cell transcriptomic sequencing to dynamically profile CXCL12–CXCR4 expression levels and functional states across different disease stages, and integrating functional blockade experiments with validation in human specimens, we will assess whether this signaling axis can serve as a therapeutic target and a biomarker of disease progression and prognosis, and explore its potential as a stage-specific, time-resolved intervention target.

Inclusion criteria

排除标准:

(1)年龄小于18周岁或大于60周岁。 (2)妊娠期、哺乳期或备孕期女性。 (3)合并其他可能导致脱发的疾病,如雄激素性脱发、瘢痕性脱发、梅毒性脱发等。 (4)合并其他自身免疫性疾病(如系统性红斑狼疮、类风湿关节炎、白癜风等)、系统性炎症性疾病、活动性感染或恶性肿瘤病史。 (5)在入组前3个月内接受过任何系统性治疗,包括糖皮质激素、免疫抑制剂(如甲氨蝶呤、环孢素)、JAK抑制剂等。 (6)在入组前1个月内,在计划取样的头皮区域使用过任何局部治疗,包括外用糖皮质激素、米诺地尔、局部免疫疗法等。 (7)已知对局部麻醉药或研究中使用的任何试剂过敏者。 (8)研究者判断不适合参与本研究的其他情况。

Exclusion criteria:

(1) Age <18 years or >60 years. (2) Women who are pregnant, breastfeeding, or planning pregnancy. (3) Concomitant conditions that may cause hair loss (e.g., androgenetic alopecia, cicatricial alopecia, syphilitic alopecia). (4) Concomitant autoimmune diseases (e.g., systemic lupus erythematosus, rheumatoid arthritis, vitiligo), systemic inflammatory diseases, active infection, or a history of malignancy. (5) Any systemic therapy within 3 months prior to enrollment, including corticosteroids, immunosuppressants (e.g., methotrexate, cyclosporine), or JAK inhibitors. (6) Any topical treatment applied to the intended scalp sampling area within 1 month prior to enrollment, including topical corticosteroids, minoxidil, or topical immunotherapy. (7) Known allergy to local anesthetics or any reagents used in the study. (8) Any other condition deemed by the investigator to make participation unsuitable.
