研究目的:

本项目拟采用行为量表和静息态功能磁共振成像技术相结合,探究左侧DLPFC不同定位方式rTMS治疗PSD患者前后抑郁症状的缓解程度及大脑功能的重组,阐明不同定位方式rTMS治疗PSD的疗效及其背后的脑机制,以期为PSD的治疗提供理论基础,预期结果有助于为患者制定更优化的治疗方案,具有重要临床价值。  

Objectives of Study:

This study aims to compare the degree of the remission of depressive symptoms, and the alterations of brain functional metrics among different locating methodology of left dorsolateral prefrontal cortex for rTMS interventions. We try to elucidate the efficacy and brain mechanisms underlying rTMS treatment for PSD, which may assist to develop more reasonable treatment plans for patients and guide clinical application. This project is of great clinical importance.

纳入标准:

Inclusion criteria

排除标准:

1.其他非血管性原因(如原发性脑肿瘤、脑转移瘤、硬膜下血肿、癫痫发作后麻痹、脑外伤等)造成的脑功能障碍;
2.既往抑郁、痴呆病史及精神疾病病史者;
3.因视力、听力、语言表达障碍、意识障碍及理解障碍(MMSE<19)无法完成检查者;
4.无法完成随访者;
5.TIA和蛛网膜下腔出血;
6.有其他神经系统疾病(帕金森、癫痫等);

Exclusion criteria:

1.brain dysfunction caused by other non-vascular causes;
2.history of depression, dementia and other psychiatric diseases;
3.MMSE < 19;
4.no show in the follow-up visit;
5.TIA or subarachnoid hemorrhage;
6.history of other neurologic diseases;

本项目拟将45名PSD患者随机分配到3种治疗方案进行干预,每种方案15名受试者。研究负责人用Matlab的命令randperm(45)生成1到45的整数随机排列后的序列,序列每个数除以3得到的余数代表三种干预方案:余数0-2分别对应方案1-3。当符合条件患者入组后,根据其入组顺序对应的随机序列的值,将其除以3的余数分配至对应的干预方案进行干预,依次类推。

Randomization Procedure (please state who generates the random number sequence and by what method):

Forty-five PSD patients will be recruited and randomly allocated into three groups for treatment, with 15 patients in each group. The principle investigator generates a series of integers from 1 to 45 with random order by Matlab code ‘randperm(45)’. Each integer of the random series will be divided by 3 and the remainder (0, 1 or 2) will be used to decide its corresponding intervention program (1, 2 and 3). After recruitment, the PSD patients will be included in one of the intervention programs. The patients will be matched with the integer in the random series generated by Matlab according to their recruiting order, and allocated to the intervention group decided by the remainder of the integer, and so on.
