研究目的：

通过多模态监测手段，系统评估三种体位（侧卧位、15°卧位、水平卧位）对老年全麻患者膈肌活动度、脑氧饱和度、苏醒质量的影响。  

Objectives of Study：

The effects of three patient positioning methods—lateral supine, 15° Trendelenburg supine, and horizontal supine—on diaphragmatic excursion, cerebral oxygen saturation, and postoperative recovery quality in elderly patients undergoing general anesthesia were systematically evaluated using multimodal monitoring techniques.

纳入标准：

Inclusion criteria

排除标准：

1.已知或怀疑困难气道（Mallampati分级IV级、甲颏距离<6cm、张口度<3cm）； 2.严重心肺疾病（NYHA心功能III-IV级、严重COPD、未控制的高血压）； 3.神经系统疾病（脑卒中病史、癫痫、帕金森病、严重颈椎病）； 4.严重肝肾功能不全（Child-Pugh C级、肌酐清除率<30ml/min）； 5.凝血功能障碍（INR>1.5、血小板<50×10^9/L）； 6.近3个月参与其他临床试验； 7.研究者认为不适合参加本研究的其他情况。

Exclusion criteria：

1. Known or suspected difficult airway (Mallampati class IV, thyromental distance <6 cm, mouth opening <3 cm); 2. Severe cardiovascular or pulmonary disease (NYHA class III-IV heart function, severe COPD, uncontrolled hypertension); 3. Neurological disorders (history of stroke, epilepsy, Parkinson's disease, severe cervical spondylosis); 4. Severe liver or kidney dysfunction (Child-Pugh class C, creatinine clearance <30 ml/min); 5. Coagulation disorders (INR >1.5, platelets <50×10^9/L); 6. Participation in other clinical trials within the past 3 months; 7. Other conditions deemed unsuitable for participation in this study by the investigator.

患者接受标准化的全身麻醉方案（诱导：丙泊酚、罗库溴铵、舒芬太尼；维持：七氟醚/丙泊酚、瑞芬太尼），术后常规佩戴自控镇痛泵。进入麻醉恢复室后，摆放于90°侧卧位，软枕支撑，拔管后维持。

Intervention：

The patients received a standardized general anesthesia protocol (induction: propofol, rocuronium, sufentanil; maintenance: sevoflurane/propofol, remifentanil), and were routinely administered a patient-controlled analgesia (PCA) pump postoperatively. Upon entering the post-anesthesia care unit (PACU), they were placed in a 90° lateral decubitus position, supported with soft pillows, which was maintained after extubation.

The patients received a standardized general anesthesia protocol (induction: propofol, rocuronium, and sufentanil; maintenance: sevoflurane/propofol and remifentanil) and were routinely equipped with a patient-controlled analgesia (PCA) pump postoperatively. After entering the post-anesthesia care unit (PACU), the head of the bed was elevated to a 15-degree semi-Fowler's position, which was maintained following extubation.

The patients received a standardized general anesthesia protocol (induction: propofol, rocuronium, and sufentanil; maintenance: sevoflurane/propofol and remifentanil) and were routinely equipped with a patient-controlled analgesia (PCA) pump postoperatively. Upon entering the post-anesthesia care unit (PACU), they were maintained in a supine position at a horizontal level, where immediate extubation was performed.
